    Our services are available to individuals and families who live in Sandoval and Bernalillo counties, New Mexico. Our programs include Infant/Toddler Services (early intervention), Autism Services, Respite Services, and Family Living.

    We are looking for a candidate to join our agency full time as a Family Service Coordinator to assist families with infants/toddlers who have developmental delays/disabilities to access and coordinate community-based early intervention services for their children.

    Duties & Responsibilities:

    • Continuous learning, training, development and improvement.
    • Conduct individual intake/enrollments, schedule evaluations, conduct interviews, facilitate, and lead team meetings, write and develop service plans, monitor service delivery and support transition to other services in accordance with the requirements of the New Mexico Family Infant Toddler Program.
    • Coordinate services with therapists, health care providers, community daycare/preschools and school personnel.
    • Support families to access other community resources, as needed.
    • Provide in-home and community-based services in Sandoval County communities (Pena Blanca, Bernalillo, Placitas, Rio Rancho, Jemez Valley) and Bernalillo County communities (Paradise Hills, Eagle Ranch, Ventana Ranch).
    • Qualify and obtain required state certifications.
    • Fulfil compliance, reporting and documentation requirements.
    • Conduct community outreach activities to identify potential service recipients
    • Share pertinent information with supervisor and team members regarding work progress, successes, barriers, concerns and other areas that may have an impact on the program services or image.
    • Maintain confidentiality of client information and records as required by HIPAA and FERPA laws, and NM Early Childhood Education and Care Department policies and procedures.
    • Build relationships with each child and family in support of service plans.
    • Occasional evening and weekend work.
    • Demonstrate sensitivity to linguistic, cultural, social, economic, individual and role differences among children and families served.
    • Travel within the state.

    Professional Requirements and Qualifications:

    • Unquestioned integrity required.

    • Outstanding interpersonal relationship skills.
    • Superior writing ability preferred.
    • Attention to detail.
    • Experience working with young children and families preferred.
    • Compliant with all applicable New Mexico Public Health Order requirements.
    • Computer experience, knowledge of or demonstrated ability to use Word, Excel, and other computer programs, as needed.
    • Bilingual (Spanish) preferred but not required.
    • Valid NM driver’s license, reliable transportation with current automobile liability policy (bodily injury liability minimum must be $100,000/300,000 and property must be $100,000) with continuous coverage.
    • A current professional liability insurance policy with continuous coverage. The agency will reimburse up to $100 per year.
    • Clearing a criminal background check required.

    Educational Requirements and Qualifications:

    Bachelor’s or master’s degree in Early Childhood Education, Education, Social Work, Human Services or another related field.

    Pay: $34,000.00 - $38,000.00 per year
