Adult Family Care (AFC) is a home-based, long-term care option for eligible Choices for Care participants who need a high level of support but who do not want to live in a nursing home or institutional setting. We work with AFC participants to find out what type of home setting they want and then match them with a contracted shared living provider whose home and lifestyle is a good fit. Adult Family Care homes provide person-centered support that is safe, supportive, and respectful of the participant’s needs, abilities, preferences, and dignity.
The service coordinator is part of a robust, collaborative team that ensures the health, safety, dignity, and quality of life for individuals served within our Adult Family Care program. The service coordinator is responsible for developing and implementing person-centered plans in accordance with federal and state requirements and for monitoring service delivery and effectiveness. The service coordinator conducts monthly home visits with assigned AFC participants to assess their wellbeing and to monitor their homes for continued safety and suitability. The service coordinator communicates effectively and ensures that all team players collaborate effectively in service to the individual’s needs.
