AdvisaCare Home Health & Hospice is looking for a Part time Master of Social Worker or (MSW)/Volunteer Coordinator. You will be working with a multidisciplinary home health and Hospice team to assess home care clients in the areas we serve in the Phoenix and surrounding area. Your duties will include complete assessments of cognition, memory, executive functioning, reasoning and planning skills to determine our client's ability to complete daily activities independently. Identify deficits that may impact their safety or ability to complete self care or other instrumental activities. Assess clients, independence levels, finances, and supportive care needs to determine available resources, make recommendations on living environment and other supportive services.
