Disease Management Case Manager coordinates the overall interdisciplinary plan of care for a patient in a disease management program, from admission to discharge. Being a Disease Management Case Manager responsibilities include but are not limited to documenting case progress, identifying health risks, and reporting the findings of the case study at appropriate intervals. Requires an associate's degree/bachelor's degree, and is licensed to practice nursing. Additionally, Disease Management Case Manager typically reports to a supervisor or manager.     Snohomish County Human Services is hiring two (2) qualified individuals to support activities for the Case Management Program. Our team supports Case Managers who provides services to elderly and disabled clients in Snohomish County.

    Position Perks:

We are a progressive workplace as we utilize a hybrid remote and office environment. Case Management Administrative Assistants must work in the office a minimum of two days per week after the training period. The county offers excellent benefits and retirement.

You will have the opportunity to engage in a variety of activities that include interactions with persons seeking assistance and service providers. You are compassionate, empathetic to those with service needs, have experience in various areas of customer service or service provision, and have strong  verbal communication skills. You understand the effects of trauma and are able to assist those who may be experiencing high levels of stress. You are collaborative and have attentive to detail and excellent organizational skills.

    BASIC FUNCTION

     Assists case management staff with complex and confidential administrative support.

    STATEMENT OF ESSENTIAL JOB DUTIES

    1. Acts as liaison between case managers and aides with clients and independent providers; initiates appropriate action regarding requests for services or complaints. 
    2. Coordinates and schedules appointments and travel arrangements; maintains multiple case management calendars. 
    3. Composes and formats letters, memos, forms and charts; proofs and edits material submitted by others for distribution; plans, organizes and coordinates meetings; takes minutes; transcribes, edits and finalizes minutes. 
    4. Maintains custody of and processes confidential client and independent provider personnel information; establishes and maintains confidential files, working files, and legal documents; screens phone inquiries and refers calls to other staff members as appropriate; processes mail and sorts according to action taken.
    STATEMENT OF OTHER JOB DUTIES
    1. Coordinates various reports and documents with other divisions, agencies, and offices as necessary. 
    2. Provides information to the public, staff, and other departments regarding policies and procedures.
    3. Coordinates special projects as assigned.
    4. Attends professional seminars, training and meetings as required.
    5. Performs related duties as required.
    An Associates degree and two (2) years increasingly complex administrative experience; OR, any equivalent combination of training and/or experience that provides the required knowledge and abilities.  Must pass job related tests.  
     
    SPECIAL REQUIREMENTS 
    A valid Washington State Driver's License may be required.

    • maintain necessary records and prepare required reports
    • manage multiple tasks/activities while meeting specific deadlines
    • maintain a high degree of accuracy with detailed information

    SUPERVISION 
    The employee reports to an administrative superior as assigned.  The supervisor establishes objectives, priorities and deadlines.  The work is reviewed through periodic meetings, status reports and by evaluation of results obtained.
     
    WORKING CONDITIONS 
    The work is performed in the usual office environment.
