May require a bachelor's degree. Additionally, Discharge Planner - Home Care typically reports to a supervisor or manager. - Description Discharge Planner - Everett Medical Ctr Colby-Everett(HR) SCHEDULE: 3 days one week and 4 days the next. Includes every other weekend, 7am-4:30pm Plans and coordinates activities related to transition planning. Develops and implements a plan of care designed to meet the patient's post hospitalization needs while ensuring continuity of care. Provides services to patients with social, mental, and cognitive impairments, per the risk stratification system. Required Qualifications: Master's Degree Social Work or Counseling Upon hire: Washington Clinical Independent Social Worker Associate License Or Upon hire: Washington Advanced Social Worker Associate License Or Upon hire: Washington Mental Health Counselor Associate License Or Upon hire: Washington Agency Affiliated Counselor Registration Experience in discharge planning, care coordination, or community resource navigation, in a clinical or community setting. Preferred Qualifications: Experience in discharge planning for medical and or surgical patients in an inpatient or outpatient setting.
