Our RN Discharge Planner is an essential part of our Medical team and is responsible for facilitating the plan for an orderly transition of patients from the acute care setting and transition to discharge. They are responsible to plan, develop, organize, implement, and evaluate all aspects of discharge planning. Implements services to patients and families; assuring that the medically related emotional and social needs are met/maintained on an individual basis. Maintain contact with referring agencies and for resources that are available. Excellent customer service, attention to detail, great telephone etiquette, computer knowledge, and the ability to multi-task in pressure/stressful situations are essential. South Dakota RN license and CPR certification is required.
The RN Discharge Planner part time position is eight hour shifts. The working conditions will be in a clean, well-lighted area, with sitting, standing, walking, bending, stooping, frequent repetitive motion of the hands/wrists involved. May be required to deal with unpleasant environmental situations resulting from patient care or patient activity. Exposure to infectious disease, body fluids, odors, and dust.
