Discharge Planner - Home Care coordinates the movement of patients between a hospital or skilled nursing facility and a home care or hospice arrangement. Reviews the patient's case and determines the best choice in services and facility to meet the medical and social needs of the patient. Being a Discharge Planner - Home Care ensures a safe transfer to a setting that meets the patient's needs and coordinates necessary services and staff required to complete the transfer. May require a bachelor's degree. Additionally, Discharge Planner - Home Care typically reports to a supervisor or manager. The Discharge Planner - Home Care gains exposure to some of the complex tasks within the job function. Occasionally directed in several aspects of the work.     The Discharge Planner has the responsibility to carry out a full range of aftercare and discharge planning service under the guidance and direction of the Inpatient Unit Team. The role of the Discharge Planners is to maintain comprehensive knowledge based of agencies, organizations, and resources within the referral community. The specific discharge planning needs of each patient shall be identified by collaboration with the unit treatment team and may include patient interviews, record reviews, and participation in network and discharge planning meetings. The Adult Discharge Planner has the responsibility of assisting the assigned social worker with securing housing, assisting in completion of Maine Care applications, case management or other community supports for those patients who require special assistance or have significant discharge planning needs, which may extend their hospitalization. The Children/Adolescent Discharge Planner will assist the assigned social worker in completing of required residential placement applications (ITRT) and case management or other community supports for those patients who require special assistance or have significant discharge planning needs, which may extend their hospitalization. The Discharge planner shall also provide reciprocal relations with county providers by serving as a liaison from the hospital and by making regular visits to various community programs when applicable. They shall also be expected to provide support to units when the social worker is unable to be there.

    The Discharge Planner will support the hospital’s Performance Improvement Plan by actively participating in multidisciplinary performance improvement teams. This individual will contribute to the ongoing continuing education programs and effectively communicate with all customers, both internal and external. Actively participate in Gemba presentations, development of team key performance indicators, data collection and problem-solving to improve quality and outcomes.

    Required Minimum Knowledge, Skills, and Abilities (KSAs)

      1. Education: Preferred Bachelor’s degree in related field.
      2. License/Certifications: LSW license or MHRT-c required.
      3. Experience: 5 years of relevant experience may be considered in substitution. Experience in healthcare required. Psychiatric setting, preferred.
