Responsibilities:
This position is responsible for providing ongoing and systematic utilization review and discharge planning efforts (in coordination with the treatment team) on all patients assigned by Director of Utilization Review; coordinating patient care; advocating for the patient and facility; and promoting quality of care.
The Discharge Planner works with Utilization Review department on certification and recertification of insurance benefits throughout the patient's stay, and assists the treatment team in understanding the barriers to discharge planning. The Discharge Planner is responsible for having a thorough understanding of the patient's treatment through communication with the treatment team. The Discharge Planner advocates for the patient's access to services during treatment team meetings and through individual physician contact. The Discharge Planner will carry a caseload, will meet with patients daily, be responsible for completing referrals, family sessions, and coordinating with outpatient providers. Discharge planners also have the responsibility of veteran and active duty patients.

Cedar Springs Hospital COVID-19 vaccination mandate:
In accordance with Colorado state public health order, all healthcare workers must be vaccinated, and the Company must verify vaccine status. Pursuant to this public health order, the following modes constitute as proof of vaccination:

an electronic entry in the Colorado Immunization Information System (CIIS); OR

an immunization record from a licensed healthcare provider who has administered a COVID-19 vaccine to an individual who provides services for the facility or agency, specifying the vaccine administered, name and title of the person who administered the vaccine, address of the location where the vaccine was administered, and the date it was administered. A
COVID-19 Vaccination Record Card (issued by the Department of Health and Human Services Centers for Disease Control & Prevention or WHO Yellow Card) which includes name of person vaccinated, type of vaccine provided, and date last dose administered would suffice.

To be eligible for an exemption based on Religious Beliefs, the worker must provide a written statement describing the worker’s sincerely held religious beliefs and how those beliefs conflict with Colorado’s August 21, 2021 Public Health Order requiring certain healthcare workers to be fully vaccinated against COVID-19. The company may request supporting documentation to further evaluate the request for religious accommodation.

To be eligible for a Qualified Medical Reason exemption, the worker must provide a written statement signed by a physician, physician assistant, advanced practice nurse, or certified nurse midwife licensed in the State of Colorado stating that the COVID-19 vaccination for the employee, direct contractor, or support staff is medically contraindicated as described in the product labeling or authorized by the FDA. This statement should not describe the underlying medical condition or disability but should indicate the probable duration of the inability to receive the vaccine.

A record of signed exemption forms and statements submitted in support of the exemption request must be maintained in a confidential HR Medical file, separate from the Personnel File.

Qualifications:
PHYSICAL REQUIREMENTS:
Position may require walking, standing sitting for prolonged periods of time.
Occasionally lifts up to 10 pounds and carries small objects from waist to chest.
Requires ability to work long periods on the phone; to hear and speak clearly over the phone.
Must be able to read and write and specifically record information and read medical records.
Regularly needs to bend, stoop and reach.
Emotional stability necessary to perform the essential functions of the job.
Vision and hearing sufficient to perform the essential functions of the job.
Overall ability to lift 50 pounds.
Requires organizational, verbal, interpersonal and customer service skills, grammar and spelling, the ability to read/comprehend written instructions and follow verbal instructions. Ability to establish and maintain effective working relationships with employees, managers, referral sources, patients/families, community resources, etc. Ability to exercise professional independent judgment in planning own work. Must be flexible with work hours to accommodate work volumes. Knowledge of utilization management and managed care environment is helpful.
EXPERIENCE/EDUCATION/LICENSURE
Bachelor’s degree in social work, psychology, counseling, or related field. Experience in case management preferred. Prefer a minimum of six (6) months' experience in a health care setting.
Experience in patient assessment, treatment planning, utilization management, and/or case management. Licensure: L.M.S.W., LPC, or other behavioral healthcare related licensure preferred. Additional Requirements: Valid CPR card. Successful completion of Satori Alteratives to Managing Aggressive Behavior (SAMA) including restraint and seclusion policies, upon employment, and prior to assisting in restraining procedures
