Position Title: Supply Discharge Coordinator Job Type: Full-time Schedule: 8 Hour shift

SUMMARY: The Discharge Coordinator works with the Management teams of all departments within HME Specialists to coordinate the communications and discharge information between the facility, internal HME department employees and patients/caregivers to ensure a timely and successful discharge of complex patients. Complex patients are defined as patients that are discharging where two or more departments within HME are involved.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned. The DC will:

• Have knowledge of various departments activities and leadership within HMES.
• Know Brightree fully and be able to assist in reviewing accounts when working on discharge coordination.
• Coordinate patient discharges from facilities throughout the state.
• Answer questions and navigate patients/caregivers through the discharge process with HMES.
• Process orders or contact discharge personnel in all departments to process orders.
• Meet and provide contact information to various facility DCP and Care Coordinators.
• Assist caregivers or patients 30 days after discharge to ensure all equipment and supplies are managed in a timely fashion and are filled properly.
• Address any issues during or after discharge for at least 30 days.

EDUCATION/EXPERIENCE:

High school diploma or general education degree (GED); 1- 2 years related experience and/or training; or equivalent combination of education and experience.

Pay: Starting $15.50 per hour
