As a Discharge Planner, you will function as a member of the interdisiplinary treatment team with primary responsibility for discharge planning and other case management duties. Essential Functions include:
Preparing treatment plans-
Organizing the paperwor, identification labels and information patients need when leaving the hospital

Scheduling after care appointments for patients with PCPs, psychiatrists, next level therapy programs, living arrangements, etc.

Coordinating releases and faxing records to providers

Data entry and chart development
-
Attending team meetings
-

Pembroke Hospital has 120 beds, offering psychiatric treatment to teens, adults and older adults. Our services are designed to assess, stabilize and treat patients by addressing their primary symptoms and problems. We emphasize family involvement as an essential component of the recovery process. Qualifications:
We require:

Associates or Bachelors degree in psychology or related human services field
-
Minimum 2 years related experience in an inpatient behavioral health setting or 3 years experience in an outpatient psychiatric setting

Experience researching and contacting public and private social service providers to assist patients in need is a plus
-
Excellent communication and organizational skills
