Duties & Responsibilities:

The examining Provider shall review and evaluate all medical examinations, tests, and findings for completeness, accuracy, and indicate the need for any additional follow - up to determine the Employee or Applicant’s ability to meet the essential duties of the position.

• One-time exam, No treatment, No follow-up, No prescriptions. Exams range from 30-90 mins. (Depending on the type of exam).
• LifeHealth is responsible for payment of services. No need to collect payment or prepare invoices.
• 1099 Independent Contractor

Qualifications

Be an MD, DO, NP or PA. Nurse Practitioners must be credentialed by either ANCC or AANPCP, PAC must be credentialed by NCCPA.
