PACE or Program of All-inclusive Care for the Elderly is a federal and state funded program that provides comprehensive, coordinated, and proactive whole-person care to help maintain and improve seniors’ health so they can live safely in their homes. HopeWest PACE provides home care, access to the day center and clinic, transportation to and from medical appointments, and activities to enhance the lives of patients and their families. This innovative program balances medical, emotional, physical, and social needs of seniors.

The registered dietitian for PACE will plan, supervise and directly participate in and document all aspects of the nutritional care of PACE participants. This person will perform nutritional assessment and provides nutrition education for PACE participants requiring intervention and will act as a resource to the health care team. The registered dietitian will coordinate serving of meals, monitor kitchen facilities, collaborate with catering vendor to ensure meals are meeting the needs of each participant.

What You Bring:

• Bachelors degree from an ADA accredited institution with a major in nutrition, Masters degree preferred
• Valid Colorado driver’s license, adequate automobile insurance and access to reliable transportation if working in the community setting
• Ability to work collaboratively and communicate effectively

What We Offer:

• Competitive pay structure starting at $30.00-$33.00 per hour, depending on experience and education
• Monday-Friday, 8am-5pm
• Mileage reimbursement for work-related driving
