PRINCIPLE CHOICE HOME HEALTHCAREPOSITION PURPOSE
Dietitian
The dietitian is a health professional who provides expert advice on nutrition and dietary management for clients. A dietitian assesses the nutritional needs of clients, develops and implements personalized eating plans, monitors and evaluates the outcomes, and educates clients on healthy eating habits and lifestyle choices.
ORGANIZATION
This position reports to the Director of Clinical Services.

- Conduct nutritional assessments and consultations for clients with various health conditions, such as diabetes, obesity, allergies, or eating disorders.
- Develop and implement individualized nutrition care plans that are evidence-based, client-centered, and culturally appropriate.
- Provide nutrition education and counseling to clients and their families, using a variety of methods, such as verbal, written, or visual.
- Monitor and evaluate the effectiveness and outcomes of the nutrition interventions, and modify the plans as needed.
- Collaborate and communicate with other health professionals, such as doctors, nurses, pharmacists, or social workers, to ensure coordinated care for clients.
- Maintain accurate and confidential records of client information, progress, and outcomes.
- Keep up to date with the latest research and developments in nutrition and dietetics and participate in continuing professional development activities.
- Participation in weekly case conferences.
- Other duties as assigned.
- Bachelor’s degree or higher in nutrition and dietetics, from an accredited institution.
- Registration or certification as a dietitian by the relevant authority.
- Active Oklahoma Medical license- in good standing
- 1-2 years recent Home Health experience preferred.
- Active Oklahoma driver’s license
- Current car insurance and registration
- Required vehicle

Specific vision abilities required by this job include close vision, color vision, peripheral vision, depth perception, and ability to adjust focus.
