Diabetes Educator educates patients on the necessary lifestyle changes involved in a diabetes diagnosis, including nutrition, medication, physical activity and glucose testing. Collects patient data to monitor health status and proactively manage the patient's condition. Being a Diabetes Educator facilitates skill development and behavior modification so that the patient is able to successfully manage the disease. Typically requires a bachelor's degree in nursing or equivalent. Additionally, Diabetes Educator typically required to be a Certified Care and Education Specialist (CDCES). Typically reports to a manager or head of a unit/department. The Diabetes Educator occasionally directed in several aspects of the work.     Schedule: Monday - Thursday
    Pay Range: $26.11 - 39.17

    Job Summary:

    Develop, maintain, practice, and teach topics of our Diabetes & Nutrition Education programs. Collaborate within a multidisciplinary team to treat and manage chronic health conditions.

    Education & License Requirements:

    Current Registered Dietitian with the Commission on Dietetic Registration (CDR). Current Licensed Dietitian (LD) in the State of Oregon. Current Certified Diabetes Care and Education Specialist (CDCES), or willing to become certified, is strongly preferred. Obtain 15 hours of diabetes-related continuing education each year.

    Minimum Qualifications & Experience:

    Previous experience in dietetics providing assessment and treatment plans to clients managing health conditions is required. Experience facilitating group presentations and group process is required. Experience in general MNT is required; experience/knowledge of providing diabetes self-management education to clients is highly desirable.

    Essential Job Functions:

      Motivate and facilitate lifestyle changes and adherence to treatment plans including, but not limited to eating behaviors and food choices.

      Maintain working knowledge and skills application of the American Diabetes Association Standards of Medical Care in providing recommendations for diabetes self-management.

      Demonstrate computer/keyboarding skills to efficiently utilize clinical electronic medical records system.

      Facilitate goal setting and problem solving with clients in groups and individually.

      Provide medical nutrition therapy and general nutrition education to various age groups.

      Express compassion while providing evidenced-based information and interventions.

      Use exceptional interpersonal, verbal, and written communication with staff, referring PCP/ Specialist, and clients.

      Assess and help manage a variety of patient health conditions.

      Document services provided and communicate with referring PCP/specialist.

      Research, plan and facilitate group presentations.

      Develop written educational materials that are appropriate for various target groups for use in classes and individual settings.

      Collect and interpret data, including statistical data.

      Balance multiple projects effectively.

      Create and evaluate additional health-based services.

    Marginal Job Functions:

    Facilitate periodic, evening diabetes classes as assigned. Light cleaning, arranging classroom and office furniture, some clerical functions. Participate in Cascade Health committees as assigned. Participate in special projects as assigned. Perform other pertinent duties as assigned.
