Diabetes Educator educates patients on the necessary lifestyle changes involved in a diabetes diagnosis, including nutrition, medication, physical activity and glucose testing. Collects patient data to monitor health status and proactively manage the patient's condition. Being a Diabetes Educator facilitates skill development and behavior modification so that the patient is able to successfully manage the disease. Typically requires a bachelor's degree in nursing or equivalent. Additionally, Diabetes Educator typically required to be a Certified Care and Education Specialist (CDCES). Typically reports to a manager or head of a unit/department. The Diabetes Educator occasionally directed in several aspects of the work. The Diabetes Educator is responsible for coordinating and conducting individual and group patient assessment and education for both diabetes and other/like chronic health conditions. Works in collaboration with physician(s) in the clinic setting to evaluate patient needs and individualize patient education. Patient education may include but is not limited to diabetes self-management (blood glucose monitoring/interpretation, carbohydrate counting, medication action/ usage and acute and chronic complications) diabetes and pregnancy, support groups, data analysis and management of patient blood glucose levels with physician orders. |
Demonstrates the knowledge and skills necessary to provide care, based upon physical, motor/sensory, psychosocial, and safety appropriate to the age of the patient served. The incumbent must provide optimal patient care through assessment, planning, implementation, and evaluation of adolescent, adult, and geriatric patients, and families.
Promotes each client’s ability to manage and improve their health through high quality education by working as a member of the multi-disciplinary team in teaching, case management, community, clinical and Renown environments.
Demonstrates the ability to apply comprehensive knowledge in the principles and techniques of chronic disease and health management.
This position does provide patient care and can either bill independently or in conjunction with a clinician patient visit.
