Diabetes Educator educates patients on the necessary lifestyle changes involved in a diabetes diagnosis, including nutrition, medication, physical activity and glucose testing. Collects patient data to monitor health status and proactively manage the patient's condition. Being a Diabetes Educator facilitates skill development and behavior modification so that the patient is able to successfully manage the disease. Typically requires a bachelor's degree in nursing or equivalent. Additionally, Diabetes Educator typically required to be a Certified Care and Education Specialist (CDCES). Typically reports to a manager or head of a unit/department. The Diabetes Educator occasionally directed in several aspects of the work. Grocery Bags, Backpack Buddies and School Pantries; to Community Health Education and Mobile Markets;
Catering and Culinary Job Training; and Community Gardening and a 14-Acre Farm; we go directly to the point
of need to empower people and overcome the burden of hunger.
The Community Health Educator (Diabetes) is responsible for facilitating monthly teleconferences, providing
individualized support, and creating an instructor guide, lesson plans, and other educational resources. This
position also works with agencies and host site partners in the Food Shuttle seven-county service area to
implement Community Health Education programs efficiently and effectively. This is a grant funded position
funded through April 2025

Teleconferences will be formatted as 3 teleconferences monthly for 1 hour with approximately 25 participants in
