The Diabetes Education Team is a consult service made up of Certified Diabetes Education Care Specialist responsible for inpatient diabetes self-management education & support by providing education for patients with history of diabetes, newly diagnosed type 1, 2, or pre-diabetes.  They work closely with the Endocrine MD’s to educate patients and families on self-care to the best possible outcome upon discharge and collaborate with providers to ensure best practice is followed appropriately for patient situation, & subsequent healthcare provider education when necessary. This group of specialized RNs advocate for patient safety regarding inpatient diabetes management, coordination of care, discharge planning, & other areas as needed.

This department covers In-hospital coverage. Hospitalized patients are seen via referral from a licensed medical provider (RN, MD, PA NP).Patients have a diagnosis of Diabetes or are pre- diabetic and need counseling. Often involves the teaching of what Insulin is and the delivery of the Insulin and also other oral agents they may be prescribed. Survival skills are taught to the patient and the patients are instructed to seek out an outpatient program for additional teaching.

Role Purpose

Provides direct patient care activities including assessment, diagnosis, planning implementation, and evaluation within the guidelines of the standards of nursing care.
