Diabetes Educator educates patients on the necessary lifestyle changes involved in a diabetes diagnosis, including nutrition, medication, physical activity and glucose testing. Collects patient data to monitor health status and proactively manage the patient's condition. Being a Diabetes Educator facilitates skill development and behavior modification so that the patient is able to successfully manage the disease. Typically requires a bachelor's degree in nursing or equivalent. Additionally, Diabetes Educator typically required to be a Certified Care and Education Specialist (CDCES). Typically reports to a manager or head of a unit/department. The Diabetes Educator occasionally directed in several aspects of the work. The Diabetes Educator RN utilizes diabetes self-management education (DSME) strategies to develop an individual assessment and education plan developed collaboratively with the individual. The plan includes:
• Educational interventions directed toward helping the individual achieve self-management goals.
• Periodic evaluations to determine attainment of educational objectives or need for additional interventions and future reassessments.
• A personalized follow-up plan developed collaboratively by the individual and educator(s) for ongoing self-management support.
• Documentation in the education record of the assessment and education plan and the intervention and outcomesJOB QUALIFICATIONS:- Associates Degree in Nursing
- Bachelor Degree in Nursing (BSN) preferred
- One (1) or more years experience as a Registered Nurse.
- Certification as Diabetes Educator (CDE) by the National Certification Board for Diabetes Educators (NCBDE) or willingness to complete within 1 year.
- Bilingual (Spanish) preferred
