Diabetes Educator educates patients on the necessary lifestyle changes involved in a diabetes diagnosis, including nutrition, medication, physical activity and glucose testing. Collects patient data to monitor health status and proactively manage the patient's condition. Being a Diabetes Educator facilitates skill development and behavior modification so that the patient is able to successfully manage the disease. Typically requires a bachelor's degree in nursing or equivalent. Additionally, Diabetes Educator typically required to be a Certified Care and Education Specialist (CDCES). Typically reports to a manager or head of a unit/department. The Diabetes Educator occasionally directed in several aspects of the work.   • Position Summary:

    Develop and implement prevention strategies as well as provide education to diabetes clients and their families. Assist in the management of the diabetes prevention fund grants. Supervise diabetes education activities in Barrow and outlying villages.


    Essential Job Functions:

    • In coordination with the SFL Program Director, develop goals and objectives for Diabetes.
    • Coordinate care for patients to facilitate best practices in diabetes prevention and management.
    • Review, select, develop and provide education and counseling materials on diabetes self-care, maintaining optimal health, and preventing complications.
    • Assess community, institutional, and family support systems and resources.
    • Collaborate with other hospital departments, medical staff, and community resources to develop joint projects that promote good health and aid in the prevention of diabetes.
    • Oversee allocations of diabetes prevention and management by the grant funds, comply with funding requirements, and submit reports in a timely fashion as required.
    • Serve as liaison with regional, state, and national organizations regarding prevention and treatment of diabetes.
    • Attend workshops and conferences for education/training purposes.
    • Adjust Diabetes Register as necessary.
    • Conduct assessment of clients and develop individualized client and family diabetes care plans consistent with cultural, social, and psychological needs.
    • Make referrals as needed both internally and externally.
    • Plan and participate in teaching and training to expand services and increase public awareness of diabetes control and prevention.
    • Maintain confidentiality and professional, ethical standards of conduct at all times.
    • Work collaboratively with multiple disciplines to provide evidenced-based diabetes care and optimize management for clients.
    • Travel to remote villages by small planes.
    • Actively participate in performance improvement activities.
    • Comply with all organizational policies regarding ethical business practices.
    • Communicate the mission and vision of Samuel Simmonds Memorial Hospital (SSMH).
    • Performs other duties as assigned.

    Qualifications:

    Education and Experience:

    • Graduate of an accredited School of Nursing; and
    • Associate’s Degree in Nursing required; Bachelor’s Degree in Nursing preferred; and
    • Current Alaska Registered Nurse License or Alaska LPN or another US state RN or LPN license with ability to obtain AK license within (6) six months; and
    • Current BLS certification; and
    • Current Certified Diabetes Educator preferred; Certification required within one year of hire; and
    • Three (3) years of recent relevant experience required.

    Licensure/Certification/Registration:

    Current Registered Nurse or Registered Dietician License (any state), Alaska Registered Nurse or Registered Dietician License required within six (6) months of hire.
