Intake Coordinator

  • Position Description:

    The Intake Coordinator serves as the gatekeeper for admission into the Shelter system. This position will screen, admit, and assess referrals for shelter programs and make determination about appropriate placement. Intake Coordinator will work closely with the Data Compliance Specialist in assuring all required documentation and paperwork is available and completed for pre-authorization process. Intake Coordinator will maintain a daily tracking sheet, logging all calls for services. Will ensure clients pursue alternative resources that prevent homelessness as possible.

    Principle Job Duties:

    1. Accepts all calls requesting shelter, pre-screens for appropriateness, makes recommendations about available resources to assist caller in avoiding homelessness

    2. When caller is not eligible at the time of call, alternative options will be offered and caller will be connected to such directly.

    3. Maintains a tracking log of all calls, certifies homelessness status, resolution if client is not accepted to shelter

    4. Completes necessary pre-admission documentation. Reviews client cases to assess and identify those in need of mental health and/or substance abuse treatment.

    5. Provides direct care and treatment for individuals, and groups which will include linking to identified resources, monitoring services received, advocating for client needs, discharge planning and follow-up. Will also provide support to the clients’ family members.

    6. Maintains client documentation per shelter requirements during screening and intake process

    7. Provides orientation to new admissions about shelter, programs, House Rules, gives tour of facility and grounds

    8. Ensure compliance with licensure requirements for agency and personally. Receives clinical supervision on a regular basis according to licensure certification requirements (1 Hour per week).

    9. Participates in continuous improvement of programs and services for the agency, making recommendations for procedure and policy changes.

    10. Performs on-call duties, as well as assists in client emergencies as needed.

    11. Provides general supervision of teams and individuals, assists with staff development and training needs, and attends necessary staff meetings and in-service trainings.

    12. Demonstrates open and honest problem and conflict resolution skills.

    13. Presents and performs in a professional manner at all times.

    Knowledge and Skills:

    Is able to demonstrate a thorough understanding of the theories, techniques, and practices of mental health and substance abuse treatment. Comprehensive knowledge of community programs, agencies and organizations. Ability to establish and maintain effective relationships with clients, their families, fellow employees and colleagues, and community groups. A commitment to our mission and the rights of those who receive our services. Excellent interpersonal and writing skills required.

    Job Type: Full-time

    Pay: $18.00 - $22.00 per hour

    Expected hours: 40 per week
