Family Team Conference Facilitator

  • Under assigned supervisor, the Family Team Conference Facilitator is responsible for implementing permanency planning practices as it relates to the process of Family Team Conferencing. The position is responsible for identifying and engaging families, facilitating meetings, and assisting with Family Finder activities.

    Job Duties

    • Assists in training, mentoring staff on practices and procedures related to family finding
      search and discovery
    • Assists the case management teams in preparation and implementation of family team
      conferences, and/or family team meetings
    • Assists in the collection and development of performance and outcome data reports, aggregation and trends for performance management and outcome management, including contract performance measures; analyze data and report findings
    • Documents activities in Florida Safe Families Network (FSFN)
    • Assists in the facilitation of family team conferences to ensure the children served by OHU are able to achieve permanency timely, efficiently and in a family centered manure
    • Responsible for providing direct assistance, consultation, and guidance to case management staff related to search and discovery of family members or non-relative supportive adult connections for the children in the system of care
    • Identifies and engages families in Family Team Conference.
    • Organizes and prepares the family support system and the case manager for the Family Team Conference.
    • Assists the family and the case manager in the development of the Family Plan using innovative, strength based and family centered practices.
    • Assists with Family Finder activities by searching for and identifying family connections and/or life-long supportive adult connections for the children in our system of care.
