Responsibilities:
Work with members in assessing health goals and implementing a Plan of Care
Assist members in addressing barriers to healthcare by providing health literacy education and addressing social determinants of health
Assist members in managing chronic behavioral health, medical and substance abuse conditions.
Link consumers to health and community resources.
Work collaboratively with health providers and other community service providers to promote primary and preventative care.
Assist consumers in reducing or eliminating the need for hospital/ER usage.
Manage all aspects of consumers electronic health records.
Promote a culture of inclusion and belonging.

Requirements:
Minimum of Associates Degree and experience working with individuals living with mental illness, substance abuse or other disabilities preferred. Proficient with Microsoft Office and other web-based programs. Prior experience with Medicaid delivery system preferred but not required. Must have reliable transportation. Valid and current New York State Driver License. Must have the ability to work in a fast-paced environment.
