Position Summary
The Palliative Care Program Coordinator is part of an interdisciplinary care team consisting of physicians, nurses, and social workers to assist in the cross-continuum approach to Advanced Care Planning and Community-Based Palliative Care. The incumbent will work closely with operational leaders across the organization including areas such as marketing and communications, physician leadership and executives to coordinate activities among the Advanced Care Planning Committee; maintain implementation plan(s); communicate team decisions; resolve issues and workflow changes and prepare briefings as needed to support the day-to-day operations of the program.Requirements
