The Home Care coordinator will be instrumental in improving the quality of life for those requiring non-skilled care while allowing them to preserve their independence and dignity in their own homes.
The Home Care coordinator evaluates, plans, and coordinates to result in a favorable health outcome for every client. The primary duty is scheduling and supporting in the ongoing oversight of caregivers, and any additional clerical support that is needed.
Other tasks may include :
May enter the patient information in the EHR, accurately and timely.
Ensures Personnel records of caregivers are complete, maintained, and compliant with regulatory requirements. Ensure staff that are not permitted to work for failure to submit, are not scheduled to see patient.
Coordinates all patient services in conjunction with the Director, Assistant Administrator, and Administrative Assistant.
Acts as a liaison among Agency’s personnel, patients and patients’ family members / caregivers.
Adheres to agency’s policies and procedures.
Work with clients and families to provide on-going assistance and information pertaining to services and care plans.
Address any caregiver relations issues that may arise and escalate to appropriate department.
Assisting with orientation of new caregivers.

Must be able to :
Office Hours are Monday through Friday 9-5.
Must be willing to take a call shift per week and be willing to do direct patient care as needed.
Must have dependable and reliable transportation during working hours.
