Compliance Coordinator - Home Care jobs in Buffalo, NY

Purpose of the Position:

The Home Care Coordinator is responsible for coordinating the care to elders and others who require assistance in the activities of daily living in their homes.

Responsibilities:

• Create and monitor service schedule using HHA Exchange
• Receive and update service authorizations for active patients
• Maintain close communication Manage Long Term Care in regards to patient's activity such as leaving service area, vacations, hospitalizations, changes in condition and information, among others.
• Assist in coordinating patient's hospital discharges, return from vacation, caregiver changes, and schedule changes as needed.
• Maintain records of all discharges per month for management review.
• Closely monitor schedules to ensure a compliant attendance verification process.
• Constant contact with patients and caregivers and providing high quality customer service, and avoid preventable discharges.

Requirements:

• High School diploma
• At least one-year experience in an office environment and customer service
• High level of communication skills, written and verbal
• Advanced skills using Microsoft Office (Excel and Word)
• Experience in homecare preferred, but not required
• Available to work full time Monday-Friday 9am-5pm
• Second language required; Preferably Bengali

Job Type: Full-time

Pay: From $19.00 per hour

Expected hours: 40 per week
