As a Veteran Care Coordinator, you will play a pivotal role in overseeing the care of current veterans while also strengthening our partnerships with VA referral providers.
Primary Responsibilities:
- Service Inquiries and Consultations: Conduct service inquiries and care consultations using our consultative sales approach, ensuring that potential clients fully understand the services we offer and how they can benefit from them.
- Client Assessment and Care Planning: Meet with referral providers, potential clients, and their families to conduct comprehensive assessments of their needs. Develop personalized care plans that address individual requirements and preferences.
- Ongoing Communication and Follow-ups: Maintain regular communication and consistent follow-ups with potential clients and families who have yet to commit to our services. Address any concerns or questions they may have, providing reassurance and support.
- Coordination of Care: Collaborate with other team members to coordinate various aspects of a client's care, ensuring seamless delivery of services and support.
- Client and Care Pro Introductions: Facilitate introductions between clients and our Care Pros, ensuring that both parties feel comfortable and informed about the caregiving relationship.
- Quality Assurance Visits: Plan and execute a schedule to ensure that each client receives at least one quality assurance visit every 45 days. Recognize opportunities to modify service plans to better support clients' ongoing needs.
- Record Keeping: Create and maintain accurate client records, documenting all quality assurance meetings and ensuring compliance with established standards.
- Conflict Resolution: Document and create action plans to resolve conflicts between clients, family members, or Care Pros, promoting positive relationships and effective communication.
- Adherence to Company Values and Policies: Reflect the core values of Home Instead and adhere to all company policies, procedures, and business ethics codes. Ensure that these standards are communicated and implemented within the team.
Secondary Responsibilities:
- Participation in Care Pro Meetings: Attend all Care Pro quarterly meetings to stay informed about organizational updates and best practices.
- Regular Attendance and Family Education Sessions: Maintain regular attendance to fulfill job responsibilities and conduct family education sessions as needed to support clients and their families.
Education/Experience Requirements:
- Bachelor's degree in a relevant field preferred.
- Two years of experience in home care, health care, or a senior-related industry required. Equivalent combinations of education and experience will be considered.
- Current CPR Certification
- Must possess a valid driver's license.
