Summary:
The Community Relations Partnership Manager identifies community partners (for-profit and not-for-profit) addressing immediate and future health needs of the communities within Bayhealth’s primary service area(s). The Community Relations Partnership Manager serves as Bayhealth’s community representative and outreach leader. The Community Relations Partnership Manager develops and leads Bayhealth’s outreach efforts as defined by Marketing & Communication’s strategic efforts in support of the Bayhealth Strategic Plan and in partnership with leaders across the health system to ensure alignment of community relations efforts that tie directly to marketing and business development strategies of the health system. ESSENTIAL FUNCTIONS & ACCOUNTABILITIES:
1. Oversees strategy and implementation of Marketing driven community outreach efforts including those that are Bayhealth owned as well as community owned collaborative efforts, including but not limited to Marketing sponsorships of community organizations, events and outreach efforts tied to Community Benefit efforts.
2. Partner across the health system and its affiliates to optimize organizational alignment and administration for efficient resourcing to decrease confusion and duplication of community efforts.
3. Develops and implements annual strategy for use of Bayhealth Mobile Care Unit in support of findings from Community Health Needs Assessment.
4. Responsible for enterprise wide CBISA tracking. Consistently inform and educate current and new leaders and employees across the organization on the use and importance of the tool. Monitor use and report on use. Develop annual Community Benefits Report partnering with Finance and the Bayhealth Foundation.
5. Collaborates with community partners to identify priority health needs within each community, directing the bi-annual Community Health Needs Assessment and developing an implementation plan.
6. Identifies community partners that address immediate and future health needs of the communities within Bayhealth’s primary service area(s). Drive strategy based on Bayhealth’s Strategic Plan.
7. Oversee and provide stewardship of Bayhealth financial contributions to community organizations and programs.
8. Manage-, train, and evaluate assigned team members Trains and evaluates staff to enhance performance, development and results. Addresses performance issues. Motivates and rewards employees.
9. Leads various Bayhealth committees and subcommittees focused on health equity, social determinants of health, and community outreach. Mobilize team responsible for representing Bayhealth in the community. Works with internal and external partners to address health inequities as part of community relations and outreach efforts.
10. Conducts landscape analysis of existing community efforts and identifies existing community programs where Bayhealth can add value or support to address priority health needs. Relays those findings to and works in partnership with the Population Health Department to identify ways to add value that will help achieve Bayhealth’s Population Health goals as well as Marketing goals to position the Bayhealth brand consistent with local consumer market research.
11. Serves as liaison and partner to the Bayhealth Population Health team in helping identify and mobilize necessary resources to address community needs. Works to support Population Health’s goals, serving as the key point of contact for Marketing & Communications to improve patient connections to social determinants of health.
12. Designs and implements strategies to improve experience of care specifically for marginalized populations in partnership with Patient Care Services, Population Health and the Bayhealth Medical Group and other divisions involved with patient experience and outreach to at-risk communities.
13. Identifies opportunities for a strategic communication/Public Service Campaign to increase understanding of social determinants of health and how they impact individual and community health.
14. All other duties as assigned within the scope and range of job responsibilities.
REQUIRED MINIMUM EDUCATION AND CREDENTIAL:
EXPERIENCE:
Required: Five years of experience in Community Engagement, Volunteer Management, Community Relations, PR, Marketing Communications, and/or Community Education Programs.
Preferred: 8-10 years of experience in Community Engagement, Volunteer Management, Community Relations, PR, Marketing Communications, and/or Community Education Programs.
