The Colorectal Cancer Alliance is a national nonprofit organization committed to ending colorectal cancer within our lifetime. We help patients, families, survivors, and caregivers navigate diagnosis and treatment options, connect them with those who can share experiences and knowledge, and identify resources to meet their needs. Together with a nation of passionate allies, we advocate for prevention, magnify support, and accelerate research to end this disease.
POSITION OVERVIEW:
With total funding support from Independence Blue Cross (IBC) in Philadelphia, the Cycles of Impact pilot is a flagship program of “Accelerating Health Equity” in Philadelphia, and it leverages the Colorectal Cancer Alliance’s “Cycles of Impact” approach for an innovative colorectal cancer prevention and screening intervention in partnership with the University of Pennsylvania Health System.
The overarching goal of the pilot is to reduce colorectal cancer mortality in Philadelphia’s Black communities. Over a three-year engagement, the Alliance aims to navigate 2,400 individuals to colorectal cancer screenings, and to catch at least 60 colorectal cancers, with a focus on Black individuals ages 45-75. We will support diagnosed patients through treatment with a personalized approach to the Alliance’s wraparound services, including our online patient and caregiver community and clinical trial matching. The expected outcome of this program will be an increase in the screening rate among Black individuals in Philadelphia by 3-3.5%.
This foundation of this CRC screening intervention is in-person navigation support – by the Alliance, regional healthcare providers and volunteer community ambassadors – which guides individuals through a digital screening quiz to determine CRC risk, and promptly escalates them to live CRC screening experts to provide access to in-home stool tests or colonoscopies, based on their personal profile.
