Community Health Director is responsible for developing plans for community-based health programs aimed at the prevention of disease and promotion of health. Recommends community health initiatives, policy implications and best practices after reviewing health literature and statistics. Being a Community Health Director identifies community health program related grant funding and develops grant proposals. Manages specific community health programs from planning and design through implementation and evaluation. Additionally, Community Health Director requires a master's degree in a related area. Typically reports to a top management. The Community Health Director manages a departmental sub-function within a broader departmental function. Creates functional strategies and specific objectives for the sub-function and develops budgets/policies/procedures to support the functional infrastructure. To be a Community Health Director typically requires 5+ years of managerial experience. Deep knowledge of the managed sub-function and solid knowledge of the overall departmental function. Role Overview: This is a professional leadership position that is responsible for providing Administrative oversight, supervision and management to a number of clinical service areas including ACCESS/Utilization Management, Intermediate Care and Emergency Services.
The individual in this position will perform as needed, essential core services such as screening, prioritization, and/or intake of consumers seeking services at the agency. This individual will provide ongoing day-to-day leadership and coordination of a multi-disciplinary team in the area of access services. Individual will supervise outreach staff in day-to-day activities and assignments with a high at-risk population. This position is also the agency liaison with the State Psychiatric Hospitals.
Additional duties of this position will include the direct supervision related policy review, monitoring and development as warranted an array of services to individuals with severe and persistent mental illness or developmental disabilities. The basic core services consist of screening, prioritization, and/or intake of consumers seeking services at the agency. This individual will provide ongoing day-to-day leadership and coordination of a multi-disciplinary team in the area of access services.
