Community Engagement and Outreach Specialist coordinates initiatives designed to promote and represent the organization and brand as a socially responsible force in the community. Position Summary
The outreach and enrollment specialists provide information and assist with insurance eligibility and enrollment within the community. They often conduct outreach and engagement to meet the needs of the underserved and vulnerable populations while maintaining privacy and security standards.
Qualifications
· Highschool diploma or equivalent.
· Associate degree in human services, health care or communication preferred.
· Ability to obtain Certified Application Counselor certificate after training period.
· Bilingual English/Spanish preferred.
Key Accountabilities
1. Educate and facilitate enrollment into coverage options available under Medicaid, Children’s Health Insurance Program (CHIP) and the Health Insurance Marketplace (healthcare.gov). Providing clarity to enrollees regarding questions and complaints related to their coverage.
2. Provide health literacy information and assistance in a fair, accurate, culturally and linguistically appropriate manner to diverse communities.
3. Conduct in-reach activities at PCHC facilities to connect patients with health services and other resources.
4. Continuously expand knowledge and understanding of community resources and services. Facilitate access to community resources including, but not limited to, locating housing, food, clothing, and relevant mental health services. Assists clients in utilizing community services, including scheduling appointments with social services agencies and the completion of applications for programs for which they may be eligible.
5. Assist clients in overcoming barriers to obtaining needed medical, dental, behavioral health care and /or social services.
6. Work collaboratively and effectively within a team. Establish positive, supportive relationships and provides feedback to other members of the team. Build and maintain positive working relationships with the clients, providers, managers, agency representatives, supervisors and office staff, from diverse cultural and socio-economic backgrounds.
7. Participate in agency and community meetings to disseminated information about PCHC.
8. Proactively identify new organizational partnerships and collaborations for client recruitment and referral. Build outcome-oriented relationships to promote access to “Coverage and Care” at PCHC.
9. Conduct process and outcome evaluation of outreach and enrollment efforts.
10. Maintain knowledge of current information and analyses regarding relevant federal, state, and local policy as well as program rules and regulations.
11. Perform other duties as assigned.
