UMMARY: Responsible for communicating goals and services of the Agency to the community. Assists potential clients with enrollment for county and state-funded attendant services. Establishes and maintains good working relationships and referral sources in the community.

- ducates and communicates goals and service offerings to the community.

- akes in-person and telephone contacts with physicians, hospitals, health care agencies and similar organizations to apprise them of the availability of the company's services and to coordinate home care.

- akes presentations to a wide range of organizations to educate the public about Agency's home health care services.

- akes visits to current clients to evaluate and discuss client satisfaction with services.

- epresents the company at appropriate meetings and organizations and acts as a liaison with other service providers in the public and private sectors.

- aintains accurate and complete records of all contacts made.

- cts as a liaison between the Agency and Facility Discharge Planning/ Social Services Department.

- onfirms that the Agency can meet the patient's needs.

- ssists physicians, nurses, social workers, and other disciplines in establishing a home care plan prior to discharge.

- nterviews patient/ family regarding home environment, current needs, and relevant psycho-social factors.

- akes referrals to Agency, including intake and background information, hospital course and care plan.

- aintains liaison relationship with hospital personnel, providing information and education on home health services.

- alls on physician offices on behalf of Agency.

- resents Agency services and offerings to physicians and their staff.

- icks up physician’s orders and delivers orders for signature during the referral process whenever possible.
