Community Development Representative implements the economic development lending activities to support or subsidize improvements in the community. Establishes and maintains relationships with groups and organizations in the community to identify lending and other business development opportunities. Being a Community Development Representative develops an understanding of public programs and community needs to ensure effective community outreach and support development. Represents the institution within the community and is a resource for information about products and services that are available. Additionally, Community Development Representative adheres to all operational procedures and complies with Community Reinvestment Act and other regulations. Requires a bachelor's degree. Typically reports to a manager. The Community Development Representative occasionally directed in several aspects of the work. Gaining exposure to some of the complex tasks within the job function.     • ducates and communicates goals and service offerings to the community. M
    • akes in-person and telephone contacts with physicians, hospitals, health care agencies and similar organizations to apprise them of the availability of the company's services and to coordinate home care. M
    • akes presentations to a wide range of organizations to educate the public about Agency's home health care services. M
    • akes visits to current clients to evaluate and discuss client satisfaction with services. R
    • epresents the company at appropriate meetings and organizations and acts as a liaison with other service providers in the public and private sectors. M
    • aintains accurate and complete records of all contacts made. A
    • cts as a liaison between the Agency and Facility Discharge Planning/ Social Services Department. C
    • onfirms that the Agency can meet the patient's needs. A
    • ssists physicians, nurses, social workers, and other disciplines in establishing a home care plan prior to discharge. I
    • nterviews patient/ family regarding home environment, current needs, and relevant psycho-social factors. M
    • akes referrals to Agency, including intake and background information, hospital course and care plan. M
    • aintains liaison relationship with hospital personnel, providing information and education on home health services. C
    • alls on physician offices on behalf of Agency. P
    • resents Agency services and offerings to physicians and their staff. P
    • icks up physician’s orders and delivers orders for signature during the referral process whenever possible. A
    • ctively participates in professional growth and development. P
    • articipates in educational programs appropriate to functions. D
    • emonstrates current knowledge of home care needs of community and Agency's services. P
    • articipate in the Quality Assessment and Improvement program. F
    • ulfills additional performance and responsibilities as assigned and as necessary.
