Pinnacle Palliative Care is seeking an energetic, empathetic, and well rounded nurse (RN or LPN) to work with our Palliative Care Team. Pinnacle Palliative Care is a Nurse Practitioner led program that specializes in treating, supporting, and managing patients who are suffering from life limiting disease processes. Unlike hospice and home health, Palliative Care is not heavily regulated by Medicare and seeks to provide an additional layer of support to patients and families regardless of their affiliation with a home health or hospice program. At Pinnacle, many palliative care cases are recommended to the palliative care program by the two large home health agencies that work within our group. Referrals do come from hospitals, skilled nursing facilities, and directly from provider offices.
The Palliative Care Clinical Intake Manager is responsible for coordinating all aspects of the referral to admission process to ensure patients and families have a clear understanding of what palliative care is/does and to set our field team up for success in preparing the patients and clinical staff for admission.
In this role, communication is critical. The individual who works in this position must be able to comfortably communicate with our interdisciplinary team (which includes physicians, nurse practitioners, nurses, and social workers), home health agencies (within the Pinnacle family and outside of the Pinnacle family), and ordering providers; while relating the wants, needs, and desires of a wide range of patient and family demographics.
Job Responsibilities:
- Communicates directly with all involved staff and family members (doctor, nurse, facility, family, etc. regarding the referral of a patient.
- Actively seeks orders for admission to palliative care by communicating with referring providers and facilities.
- Responsible for communicating with nurses completing needs assessments to coordinate and schedule timely admissions of patients in need of our services.
- Records any pertinent medical history and assists in obtaining proper medical records that may be relevant to determine eligibility or primary focus for our palliative team.
- Communicates updates and changes in patient status to the interdisciplinary team as well as referring groups as well as the group of clinicians potentially already treating these patients.
- Establishes, expands, and maintains, positive working relationships with current and potential referring entities to bring awareness to the community of what palliative care can offer.
- Maintains compliance by documenting accurately and timely within the organizations electronic medical record.
- Comfortable discussing goals of care, advanced directives, and palliative care philosophy.
- Communicates with leadership, sales team, and clinical staff daily regarding status of pending/possible referrals.
Job Requirements:
- Active Licensed Practical Nursing or Registered Nursing license in Louisiana
- At least 2-3 years of experience in healthcare.
- Preferred if experience is in palliative care or hospice.
- Computer experience including knowledge of Microsoft and electronic medical record keeping.
Expectations:
-This role is a full time in office position that occasionally visits patients in their homes.
-40 hours per week.
