CareLink is a nonprofit agency that provides resources for older people and their families in central Arkansas. We are the Area Agency on Aging for central Arkansas. MINIMUM QUALIFICATIONS
- Completed two years of full-time study at an accredited institution of higher learning; or
- Completed one (1) year of full-time study at an accredited institution of higher learning and has one (1) year of full-time employment in a supervisory capacity in a healthcare facility or community-based agency; or
- Has a high school diploma or general equivalency diploma and has two (2) years full-time employment in a supervisory capacity in a healthcare facility or community-based agency.
- Reliable transportation with valid AR driver’s license and auto liability insurance with increased coverage limits
- Home health or home care experience preferred
KNOWLEDGE, SKILLS, ABILITIES
- Knowledge of home care programs and governing rules
- Ability to drive car for home visits; walk up and down steps to client homes; navigate uneven surfaces
- Ability to review documentation and instruct caregivers on activities and tasks to perform according to an individualized service plan
- Ability to communicate orally and in writing using proper grammar and correct spelling
- Ability to maintain organized records and files
- Ability to effectively supervise the work of others
- Ability to problem solve and make decisions
- Ability to operate technical devices that include, but not limited to, smart phone, laptop computer, printer, fax, scanner, and software programs
- Ability to work autonomously and maintain program integrity and confidentially
DUTIES AND RESPONSIBILITIES
- Makes in-person supervisory visits in accordance to the recommended frequency on the client’s service plan
- Observes risk factors for client during home visit and reports changes to the agency Home Care Nurse Supervisor
- Determines general status of client and level of satisfaction with service
- Checks for other means of support in temporary absence of caregiver, e.g., friend, family member, neighbor, church
- Completes weekly chart reviews on clients.
- Monitors performance and attendance of caregiver through home visits and telephone contact with the client, direct observation, and telephony (Generations)
- Confirms caregiver time in Generations daily/weekly
- Assists scheduling with finding caregivers to staff clients
- Completes supervisory visit forms and obtains signatures as required
- Keeps chart current/files all documents in Laserfiche timely and accurately
- Documents every contact with client, family member/caregiver, doctor
- Reviews reports from Generations and from HC Manager for accuracy and provides feedback to HC Manager
- Review caregiver notes and document the record review at least monthly; if necessary, provide additional instructions to the caregiver and document the reasons for further instructions and the caregiver’s feedback to the instructions
- Recommends changes to procedures in writing
- Ensures compliance of work functions with funding requirements, licensing if applicable, agency policy and other external requirements
- Identifies and recommends solutions to problems
- Responds to customer questions/complaints timely and professionally
- Reviews and submits corrections to monthly/quarterly reports in a timely manner
- Participates in PEER review and provides explanations for review findings
- Participates in community outreach when requested
- Participates in new caregivers recruitment, hire, and retention.
- Practices a supervisory style which contributes to a positive and productive work climate for all employees.
- Works within policies and procedures for personnel actions, e.g., performance evaluations, counseling, leave approval, selection, orientation, etc.
- Regularly meets with staff individually and as a group to provide feedback, problem-solve, and to communicate information
- Regularly utilizes positive reinforcement to develop and motivate employees
- Supervises Service Coordinators and staffing and associated tasks are being completed in a timely manner.
- Responsible for approving time and leave for Service Coordinators
- Stays abreast of current trends, changes, etc. in specialty field
- Obtains the required number of training hours annually if applicable
- Participates in agency sponsored events and health fairs
- Provides departmental orientation to new staff or interns
- Attends relevant training provided by the agency
Salary Range: $39,696-$48,306
