Klickitat Valley Health is looking for a Clinical Pharmacist to join our dynamic and dedicated Pharmacy team. This position offers 40 hours a week. The Pharmacy is staffed Monday through Friday 9 am-7 pm and Saturday 9 am-2 pm.

Klickitat Valley Health is a rural, critical-access hospital located in scenic Goldendale, Washington only two hours from Portland along the breathtaking Columbia River Gorge. The Clinical Pharmacist plays a vital role in patient care treatment through drug therapy services provided at KVH. These services are age appropriate, for neonates, pediatric patients, adolescents, adults and geriatric patients, performed when needed through direct contact with the patient, and/or in-direct association with or in support of health care providers.
The Pharmacist Integrates the provision of pharmaceutical care with other elements of the patient care process to provide a collaborative, cooperative effort with other members of the health care team to assure optimal drug therapy outcomes. ESSENTIAL FUNCTIONS / RESPONSIBILITIES (but not limited to):
Pharmacist will demonstrate clinical competence as required by program needs
Reviews, verifies, and processes provider medication orders ensuring appropriate therapy based on the patient’s
condition and overall medical history.
Provides direct patient care, including medication therapy management, patient counseling and therapeutic drug
monitoring.
Documents pharmaceutical care in patient’s medical record.
Collaborates with healthcare teams to provide evidence-based medication recommendations.
Participates in interdisciplinary rounds.
Assists in the management and reporting of adverse drug events and medication errors.
Educates health care practitioner regarding drug therapy.
Participates in drug use evaluations, quality assurance and continuous quality improvement programs to enhance medication
safety and patient care.
Participates in formulary control and maintenance.
Participate in the training and mentoring of pharmacy interns, residents, and other healthcare professionals.
Oversees the preparation and dispensing of medications including sterile compounding.
Monitors and manages drug inventories to ensure timely and effective patient care.
Maintains accurate inventory records on narcotics as required by law.
Responsible for drug inventory out date checks.
Assures that proper charging documentation is completed correctly for services provided.
Responsible for completion of all assigned work procedures during the shift without interference to quality of work
performed.
Delegates other responsibilities in order to keep patient care the number one priority.
Regularly assists other pharmacists and pharmacy technicians as time allows and as requested.
Responds to changes in work load as necessary. Uses time in productive manner-assists technicians & other
pharmacists, restocks, cleans, orders, organizes, special projects, etc.
Contributes to the development and maintenance of hospital medication policies and procedures
Participates in hospital committees, meetings, and initiatives.
Ensures compliance with Federal, State and local regulations regarding drug dispensing, storage, and record
keeping.

Maintains certification and demonstrates knowledge of required safety, security and emergency programs required
by department or hospital.
