Clinical Pharmacist directs and develops research of existing and new prescription drugs and conducts utilization reviews to develop guidelines for usage, prior authorization, and updates to formulary. Monitors patient data on drug therapies outcomes, reactions, and errors. Being a Clinical Pharmacist provides education and outreach to medical staff on prescribing guidelines, alternative drug therapies, and risk mitigation. Administers a prior authorization program. Additionally, Clinical Pharmacist requires a doctorate degree in pharmacy (Pharm.D.). Typically reports to a manager or supervisor. Requires Registered Pharmacist (RPh). MINIMUM QUALIFICATIONS:
EDUCATION, CERTIFICATION, AND/OR LICENSURE:
1. Bachelor of Science in Pharmacy OR Doctor of Pharmacy (Pharm.D.) from an ACPE accredited school of pharmacy.
2. Must possess current license to practice pharmacy in West Virginia or be license eligible. Licensure must be completed within 90 days of hire. If licensure is not obtained within 90 days of start date, associate will be notified that they will be released from their employment.
3. Obtain certification in Basic Life Support within 60 days of hire date.

1. Provides Pharmacotherapy for all covered patients.
2. Makes therapeutic recommendations.
3. Pharmacokinetic interventions.
4. Provides current evidence based medication information to all patient care providers.
5. Maintains service availability thru Unit Specific Pharmacist Program for nursing, pharmacy, and allied health professionals.
6. Takes Call per Call Schedule (on call and code response).
7. Completes IV to PO Conversion per current policy.
8. Precepts Pharm D students.
9. Mentors Pharmacists.
10. Completes ADR forms for covered patients.
11. Completes Anticoagulation monitoring daily and document.
12. Participate in prescribing and monitoring of high-cost, problem prone medications.
13. Documents Clinical Activities in RXLink.
14. Participates on Collaborative Practice Group when applicable.
15. Participates in formulary management via P&T Work Team (including MUE/TI. Class review, and new drug evaluation) as requested. Supports maintenance of formulary streamlining/operations.
16. Develops and implements pharmacy policy and procedure for achieving compliance with national patient safety goals and regulatory requirements under the direction of the Director. (Examples are, but not limited to pharmacokinetics, therapeutic interchange).
17. Submits articles for publication in the "Capsule" pharmacy newsletter to medical staff.
18. Signs the BOP Journal daily.
19. Completes inpatient competency evaluation as required. (Policies, procedures, staffing, IV prep, ACLS, etc.).
20. Participates in community projects for hospital and community well-being expected.
21. Directs and delegates the tasks associated with the central pharmacy when assigned to the central pharmacy when assigned to the central pharmacy to assure prompt and accurate delivery of medications to the patient areas.
22. Responds to all code blue and code triage emergencies and assist with medication needs.
