Job Description:
Essential Job Functions:
- Collaborate with providers to assess patient's medication needs, develop treatment plans, and provide comprehensive pharmaceutical care.
- Review patient's medication regimens, taking into consideration possible drug interactions, allergies, and contraindications, and make appropriate recommendations to ensure safe and effective therapy.
- Monitor patient's therapeutic response to medications, identify and manage drug-related problems, and adjust therapy as necessary.
- Educate patients and caregivers on medication use, proper administration, potential side effects, and the importance of adhering to prescribed therapies.
- Contribute to the development and implementation of clinical pharmacy services and protocols to improve patient care quality and safety.
- Participate in multidisciplinary teams and provide drug therapy expertise, including medication reconciliation and appropriate drug selection, to ensure optimal patient outcomes.
- Conduct medication regimen reviews to identify potential cost-saving opportunities, therapeutic interchange opportunities, and optimize medication utilization.
- Stay updated on the latest advancements in pharmaceutical science and practice, attend professional conferences, and engage in continuing education to maintain professional competence.
- Foster effective relationships with healthcare providers, patients, and the community to promote collaborative and patient-centered care.
- Track and evaluate programs for operational, quality, and financial efficiency and routinely benchmark against local and national best practices.
Knowledge, Skills, and Abilities:
- Strong clinical knowledge and understanding of medication therapy management, drug interactions, pharmacokinetics, and pharmacodynamics.
- Excellent communication and intrapersonal skills to effectively collaborate with healthcare professionals, patients, and caregivers.
- Ability to work independently and as part of a multidisciplinary team.
- Proficient in using pharmacy information system(s) and clinical decision support systems.
- Commitment to maintaining patient confidentiality and privacy.
Experience and Education Qualifications:
- B.S. or PharmD degree from ACPE accredited institution
- MTS certification
- Successful completion of PGY-1 residency or like professional experience
- BCPS Pharmacotherapy or Psychiatry preferred, but not required
Supervisory Requirements:
Employment Requirements:
- Successful completion of background check including criminal record, and abuse/neglect.
- Completion of New Hire Orientation at the beginning of employment.
- All training requirements including Relias Learning at the beginning of employment and annually thereafter.
- Clear TB/ CDS if required by program
