Clinical Pharmacist directs and develops research of existing and new prescription drugs and conducts utilization reviews to develop guidelines for usage, prior authorization, and updates to formulary. Monitors patient data on drug therapies outcomes, reactions, and errors. Being a Clinical Pharmacist provides education and outreach to medical staff on prescribing guidelines, alternative drug therapies, and risk mitigation. Administers a prior authorization program. Additionally, Clinical Pharmacist requires a doctorate degree in pharmacy (Pharm.D.). Typically reports to a manager or supervisor. Requires Registered Pharmacist (RPh).     FT ; 2045-0700 This position is 7 days on, 7 days off, will consider new graduates. Possible float to CMC, Mercy, Union, Cabarrus, and Pineville

    Provides pharmaceutical care through staffing and clinical rounds. Reviews and monitors physician orders for drug therapy and monitor this therapy for drug choice, dosage, drug interactions, adverse drug reactions, and formulary adherence. Serves as a drug information resource to nurses, physicians, and other healthcare workers. Assists with the implementation of identified pharmacy initiatives as applicable to the pharmaceutical care center. Serves in a Charge Pharmacist role and is responsible for all patient care operations and problem-solving during the overnight shift.



    Essential Functions

    • Coordinates, oversees, and participates in all patient care activities, on-site supervision, problem-solving and operations decisions working third shift.
    • Uses effective patient education techniques to provide counseling to patients and caregivers, including information on medication therapy, adverse effects, compliance, appropriate use, handling, and medication administration.
    • Exhibits highly effective verbal communication and interpersonal skills with visitors and other healthcare team members to establish working relationships that foster optimal quality of patient care.
    • Understands and can utilize the automated dispensing equipment in the practice area.
    • Alerts appropriate team member of drug shortages and problems (i.e. out of date drugs, damaged packaging).
    • Determines the presence of medication therapy problems in patient’s current medication therapy.
    • Participates in Quality Assurance and Performance Improvement activities.
    • Interprets the appropriateness of a medication order before preparing or permitting the distribution of the first dose. Utilizes pharmacy support team members effectively, checks the accuracy of the work, and prioritizes the workload and organize the work flow.

     

    Physical Requirements
    Works primarily in the pharmacy. Delivers medications to nursing units. Requires considerable walking, standing, pushing mobile cart of 50 pounds maximum and lifting 25 pounds of material. May be required to frequently stretch, bend, twist, squat, or kneel to perform job. Exposure to cancer chemotherapy agents. Otherwise, hazards are very minor such as minor cuts, bruises, and non-contaminated needle pricks. Exposed to various conditions throughout the workday such as walking on hard surfaces, climbing stairs and changes in temperature and humidity. Must be able to perform manipulative skills such as writing, typing, and data entry into the computer and automated dispensing machines. Must have use of both hands, be able to see clearly and read small type such as that on unit-dose medication packages and syringes. Must be able to speak and be understood by others. Must be able to hear normal sounds with moderate background noises.



    Education, Experience and Certifications

    Bachelor's Degree in Pharmacy or a PharmD from an accredited school of pharmacy required. Licensed in the state of North Carolina or South Carolina as applicable or eligible for reciprocity required. Previous hospital pharmacy experience preferred.
