Clinical Pharmacist directs and develops research of existing and new prescription drugs and conducts utilization reviews to develop guidelines for usage, prior authorization, and updates to formulary. Monitors patient data on drug therapies outcomes, reactions, and errors. Being a Clinical Pharmacist provides education and outreach to medical staff on prescribing guidelines, alternative drug therapies, and risk mitigation. Administers a prior authorization program. Additionally, Clinical Pharmacist requires a doctorate degree in pharmacy (Pharm.D.). Typically reports to a manager or supervisor. Requires Registered Pharmacist (RPh).     Matrix Providers is hiring a Clinical Pharmacist to join our team of talented professionals who provide healthcare services to our Military Service Members and their families at BHC Oceana at Medical Home Port in Virginia Beach, Virginia.

    • Employment Status: Full-time
    • Compensation: This is a salaried position, paid bi-weekly.
    • Schedule: Services shall be provided Monday through Friday between the hours of 7:00 a.m., and 7:00 p.m. Shifts will generally be scheduled for an eight and a half (8.5) to nine (9) hour period, to include an uncompensated thirty (30) minute to sixty (60) minute lunch break, respectively. In no instance will the healthcare worker be required to provide services above eighty (80) hours per two-week period. The clinic is open on the following Federal Holidays: Martin Luther King Jr's Birthday, President's Day, Columbus Day, and Veteran's Day.

    Requirements: The Pharmacist must have and meet the following:

    • Degree: Bachelor's degree in pharmacy.
    • Education:
      • Graduate from a college or university accredited by the Accreditation Council on Pharmaceutical Education (ACPE) approved continuing education of at least 15 hours which maintains pharmacy skills and knowledge in the preceding two (2) years.
      • Possess one of the following:
        • Completion of an accredited Pharmacy Residency Program
        • Possess board certification (from the Board of Pharmacy Specialties) in Pharmacotherapy Ambulatory Care Pharmacy or Psychiatric Pharmacy
        • Master's degree in Medication Therapy Management (MTM).
        • Possess two (2) years of clinical practice managing and modifying drug therapy according to written protocols or collaborative practice agreements between the pharmacist and individual physician who are responsible for the patient's care or patient-centered medical home experience within the preceding three (3) years.
        • Have held clinical privileges as a clinical pharmacist within the preceding thirty-six months (36) months in a military treatment facility.
      • Possess experience in the use of pharmacy computer systems such as the Navy TRIPHARM Computer System, the Composite Health Care System (CHCS), or retail pharmacy computer systems.
    • Licensure: Current, full, active, and unrestricted license to practice as a pharmacist.
    • Certification: Basic Life Support (BLS) from American Heart Association or American Red Cross. Web-based classes do not meet these standards.
    • Recommendation Letter: Provide two (2) letters of recommendation written within the last two (2) years attesting to the healthcare worker's clinical skills. The letters must be from practicing Pharmacists who have personally observed and evaluated the healthcare worker and can attest to the healthcare worker's clinical skills in the care of patients. Reference letters shall attest to the healthcare worker's communication skills and ability to relate to patients as well as professional and other interpersonal skills among staff members and must include the name, title, phone number, date of reference, address, and signature of the individual providing reference.

    Job Summary:

    • Perform a full range of pharmacist procedures
    • Deliver comprehensive pharmaceutical services within the facility's personnel, supply, and equipment capabilities.
    • Counsel patients regarding the appropriate use of medications.
    • Review patient profiles at the time of dispensing to monitor medication therapy closely.
    • Ensure prescription directions for use, dose, medication interactions, therapeutic duplications or overlaps, allergy information, age-specific dosage, and other pertinent data are appropriate for individual patients. Intervention with providers shall be made for noted discrepancies of the above-listed items.
    • Compound medications as required.
    • Ensure the accuracy of all pharmaceutical products prepared or processed by technical staff.
    • Supervise inventory control of all pharmaceuticals in the pharmacy, especially scheduled pharmaceuticals, and ensure pharmacy security. Maintain appropriate records and security of all scheduled controlled substances according to applicable regulations.
