Clinical Pharmacist directs and develops research of existing and new prescription drugs and conducts utilization reviews to develop guidelines for usage, prior authorization, and updates to formulary. Monitors patient data on drug therapies outcomes, reactions, and errors. Being a Clinical Pharmacist provides education and outreach to medical staff on prescribing guidelines, alternative drug therapies, and risk mitigation. Administers a prior authorization program. Additionally, Clinical Pharmacist requires a doctorate degree in pharmacy (Pharm.D.). Typically reports to a manager or supervisor. Requires Registered Pharmacist (RPh). Ambulatory Care Clinical Pharmacist I

    POSITION DESCRIPTION:

    Provides ambulatory clinical pharmacy services (including disease management and comprehensive medication review and Population Health Management under a collaborative practice agreement at multiple sites within the organization.

     

    ESSENTIAL JOB FUNCTIONS:

    1. Provides pharmaceutical care by participating in patient care with the medical staff during outpatient clinic visits. This includes designing, implementing and monitoring patient-specific pharmacotherapy; promoting safe, evidence-based and cost-effective use of drug therapy.
    2. Performs Population Health initiatives including reaching out to patients, based on pre-determined clinical criteria to enroll them in pharmacist services in order to meet provider-delivered care management goals.
    3. Performs Population Health initiatives, including reaching out to patients based on clinical, medication or payer needs identified.
    4. Accurately documents interventions and outcomes in drug therapy quality improvement efforts.
    5. Implements and maintains clinical pharmacist services at multiple ambulatory care sites within IHA.
    6. Works collaboratively with other clinicians, which may include care managers and behavioral health care managers, as well as healthcare providers to implement and maintain innovative disease state management program and clinical pharmacy services.
    7. Collaborates with pharmacists within IHA in information and best practice sharing.
    8. Assists in the implementation of quality improvement processes to monitor the effectiveness and quality of pharmacists’ services.
    9. Provides drug information and education to medical staff and patients.
    10. Conducts or participates in drug use projects, cost reduction/management initiatives, and quality improvement activities.
    11. Evaluates and manages day to day workflow and adjusts as needed to increase efficiencies.
    12. Maintains regular contact with the staff at each practice location. Makes attempts to attend provider and/or staff meetings whenever possible.
    13. Prepares for and is available for site audits.
    14. Functions as a role model and an educational resource.
    15. As needed, serves as a primary preceptor for pharmacy students and/or pharmacy residents.
    16. Supports other offices, attends required meetings and training, and participates in committees as requested.
    17. Assists with special projects, performs other duties as assigned and works within the scope of Pharmacist and Controlled substance licensure.

    8. Serves as a role model by demonstrating exceptional ability and willingness to take on new and additional responsibilities.  Embraces new ideas and respects cultural differences.
    9. Uses resources efficiently.
    10. If applicable, responsible for ongoing professional development – maintains appropriate licensure/certification and continuing education credentials, participates in available learning opportunities.

    ESSENTIAL QUALIFICATIONS:

    EDUCATION:  Doctor of Pharmacy/Pharmacology degree required. Completion of self-management support training preferred.

    CREDENTIALS/LICENSURE:   Valid, unrestricted license as a Pharmacist by the State of Michigan; Valid, unrestricted State of Michigan Controlled Substance License. Valid CPR certification.

    MINIMUM EXPERIENCE:   Five (5) years of relevant experience is preferred. A Post-Graduate Year 1 (PGY1) Pharmacy Practice Residency or Post-Graduate Year 2 (PGY2) Specialty Residency can substitute for relevant experience. Board Certified Ambulatory Care Pharmacist (BCACP) preferred and BCACP Board-Eligible considered.

     

    POSITION REQUIREMENTS (ABILITIES & SKILLS):

    1. Knowledge of ambulatory patient care procedures and organizational policies related to position responsibilities.
    2. Strong knowledge base in the outpatient management of diabetes, hypertension, and hyperlipidemia.
    3. Proficient/knowledgeable in medical terminology.
    4. Ability to perform mathematical calculations needed during the course of performing basic job duties, i.e.: calculating proper dosages for insulin.
    5. Proficient in operating a standard desktop and Windows-based computer system, including but not limited to, electronic medical records, email, e-learning, intranet, Microsoft Word and Excel, and computer navigation.  Ability to use other software as required while performing the essential functions of the job.
    6. Excellent communication skills in both written and verbal forms, including proper phone etiquette.  Ability to speak before groups of people, either in-person or virtually. 
    7. Ability to work collaboratively in a team-oriented environment; displays courteous and friendly demeanor. Collaborates clinical care ideas across multiple patient care services and communicates effectively as a patient advocate.
    8. Ability to exhibit a professional and positive image and work effectively with various levels of organizational members and diverse populations including IHA staff, providers, family members, insurance carriers, vendors, external customers and community groups. 
    9. Ability to cross-train in other areas of practice in order to achieve smooth flow of all operations.
    10. Good organizational and time management skills to effectively juggle multiple priorities and time constraints.
    11. Ability to exercise sound judgement and problem-solving skills, specifically as it relates to resolving medication reconciliation issues.
    12. Ability to handle patient and organizational information in a confidential manner.
    13. Knowledge of the compliance aspects of clinical care and patient privacy and best practices in medical office operations.
    14. Ability to drive to multiple sites within a week or same-day.
    15. Successful completion of IHA competency-based program within introductory and training period.
