CLINICAL OUTCOMES MANAGER manages the collection and analysis of clinical outcomes data to develop clinical process improvement initiatives. Oversees staff who gather data on performance metrics and facilitates the review of clinical program effectiveness. Being a CLINICAL OUTCOMES MANAGER provides advanced analyses and reports on clinical outcomes data. Evaluates and summarizes patient data to ensure that care is provided in accordance with clinical guidelines and organizational standards. Additionally, CLINICAL OUTCOMES MANAGER identifies areas for improvement and assists leaders/physicians in developing strategies to improve performance. Requires a bachelor's degree in nursing or healthcare related area. May require a Registered Nurse (RN) license. Typically reports to a director. The CLINICAL OUTCOMES MANAGER typically manages through subordinate managers and professionals in larger groups of moderate complexity. Provides input to strategic decisions that affect the functional area of responsibility. May give input into developing the budget. To be a CLINICAL OUTCOMES MANAGER typically requires 3+ years of managerial experience.     SUMMARY
    The Case manager works under the supervision of the Regional Director of Clinical Services and is responsible for implementing client services in a manner that addresses the Talbot Health Services mission and ensuring they are provided consistently with the organizations vision and values. This is an hourly position with hours and a schedule established to complete the required duties. This position is not exempt from wage and hour regulations as defined by the U.S. Department of Labor and is therefore granted protections under the Wage and Hour Division of the Fair Labor Standards Act.
    EDUCATION, EXPERIENCE, LICENSE, CERTIFICATION
    Required
    • Active Ohio CDCA license
    • High school diploma, GED or equivalent
    • Three years of relevant experience
    • Valid Ohio driver’s license and good driving record
    Preferred
    • Bachelor’s degree in a related field
    • CPR/First Aid/AED Training
    • Crisis Prevention/De-escalation Training

    ESSENTIAL DUTIES AND RESPONSIBILITIES
    • Monitor the needs of client’s mental illness or substance use and physical symptoms as well as their response to treatment; communicate these changes to the treatment team to ensure immediate and appropriate interventions are provided.
    • Work with clients and their families to identify needs and connect them with resources to meet those needs.
    • Coordinate community resources including school, probation officers, referral sources and outpatient mental health providers to ensure continuity of care.
    • Facilitate appointments to ensure overall physical well-being including basic mental health and substance use needs.
    • Achieve productivity goals as assigned.
    • Perform duties in accordance with applicable professional ethics and standards and established agency policies.
    • Comply with all state and federal laws regarding protected health information.
    • Contact clients within 24 hours of case assignment.
    • Offer clients an appointment within 48 hours of initial contact.
    • Complete documentation within 48 hours of provided service(s).
    • Schedule all client appointments weekly, at minimum, with the goal being two visits per week.
    • Coordinate with the treatment team bi-weekly, at minimum.
    • Complete basic living skills assessments on all clients.
    • Follow all best practices.
    • Mandatory participation in weekly supervision.
    • Maintain client chart with up-to-date client information (demographics)
    ADDITIONAL DUTIES AND RESPONSIBILITIES
    1. Perform all duties in a manner that’s friendly, considerate, and aligned with the Agency’s vision and values in accordance with applicable professional ethics and standards and agency policies. 2. Comply with all state and federal laws regarding PHI.
    3. Organize and plan work for maximum efficiency and effectiveness.
    4. Utilize IT systems and software programs in accordance with agency policy.
    5. Utilize time off in a manner consistent with agency policy.
    6. Perform other duties as deemed necessary and appropriate by supervisor.
    7. Abide by all clinical policies and procedures.
    8. Attend all staff meetings unless extenuating circumstances are prevalent.
    9. Abide by the professional ethics of the individual’s professional discipline and organization.
    10. Maintain full compliance with state and federal rules and guidelines regarding treatment.
    11. Adhere to all rules of Federal confidentiality.
    12. Adhere to all requirements established by funding sources for maintaining case records.

    • Ability to learn and utilize the agency’s electronic tools effectively and efficiently within 30 days of hire.
    • Demonstrate a current knowledge of mental health and substance use practices.

    • Demonstrate knowledge of community resources and partnerships available.
    • Demonstrate the ability to effectively function as an agency team member.
    • Demonstrate the ability to effectively manage multiple tasks concurrently.
    • Possess and demonstrate effective organizational skills and the ability to work independently.
    • Demonstrate the ability to customarily and regularly exercise discretion and good professional judgment

    • Must be able to lift to 15 pounds at a time.
    • Ability to perform job responsibilities in facilities or community-based residences and buildings with multiple levels and without elevator or mechanical support.
