CLINICAL OUTCOMES MANAGER manages the collection and analysis of clinical outcomes data to develop clinical process improvement initiatives. Oversees staff who gather data on performance metrics and facilitates the review of clinical program effectiveness. Being a CLINICAL OUTCOMES MANAGER provides advanced analyses and reports on clinical outcomes data. Evaluates and summarizes patient data to ensure that care is provided in accordance with clinical guidelines and organizational standards. Additionally, CLINICAL OUTCOMES MANAGER identifies areas for improvement and assists leaders/physicians in developing strategies to improve performance. Requires a bachelor's degree in nursing or healthcare related area. May require a Registered Nurse (RN) license. Typically reports to a director. The CLINICAL OUTCOMES MANAGER typically manages through subordinate managers and professionals in larger groups of moderate complexity. Provides input to strategic decisions that affect the functional area of responsibility. May give input into developing the budget. To be a CLINICAL OUTCOMES MANAGER typically requires 3+ years of managerial experience.     Home Town Health Care is looking for a passionate Registered Nurse with home health or leadership experience to help lead our growing team.  This position requires skills in planning and organizing nursing care. The Clinical Manager will use advanced nursing clinical skills while maintaining electronic medical records and charts, electronically searching and preparing reports, teaching other nursing-related skills, and maintaining effective working relationships with others. This position combines the activities of direct and indirect care for pediatric, adolescent, adult, and geriatric patients. 


    Duties Include: 

    Receives all referrals and assigns the patients to the appropriate nursing staff for the most efficient travel and care

    Completes the Plan of Care preparation sheet on all SOC

    Plans for patient discharge

    Conducts weekly case conference 

    Enters diagnosis and medication information into the EMR

    Assist the RN Case Manager to create a skilled plan of care, scheduling their visits and ensuring a correct master schedule as needed 

    Ensures that all disciplines are scheduled based on plan of care and order frequencies and that visits are made timely

    Participates in patient care with regularly scheduled visits and PRN visits as needed

    Oversee and assist the office to schedule the HHA accordingly 

    Completes time audits 

    Runs regular reports

    Monitors daily schedule to ensure visit documentation is completed 

    Ensures correction(s) from Quality Improvement and Chart Audit are completed timely according to policy

    Understands the legal implications or regulations and laws regarding proper documentation

    Participates in in-service programs and manager meetings

    Monitors supply inventory

    Requirements

    Bachelors or Associates Degree in Nursing from an accredited nursing program

    Registered Nurse, currently licensed in the state of Kansas

    Valid state driver's license

    Public Health Nursing experience desirable with a minimum of two (2) years in a community health setting.
