CLINICAL OUTCOMES MANAGER manages the collection and analysis of clinical outcomes data to develop clinical process improvement initiatives. Oversees staff who gather data on performance metrics and facilitates the review of clinical program effectiveness. Being a CLINICAL OUTCOMES MANAGER provides advanced analyses and reports on clinical outcomes data. Evaluates and summarizes patient data to ensure that care is provided in accordance with clinical guidelines and organizational standards. Additionally, CLINICAL OUTCOMES MANAGER identifies areas for improvement and assists leaders/physicians in developing strategies to improve performance. Requires a bachelor's degree in nursing or healthcare related area. May require a Registered Nurse (RN) license. Typically reports to a director. The CLINICAL OUTCOMES MANAGER typically manages through subordinate managers and professionals in larger groups of moderate complexity. Provides input to strategic decisions that affect the functional area of responsibility. May give input into developing the budget. To be a CLINICAL OUTCOMES MANAGER typically requires 3+ years of managerial experience.     Responsibilities include:
    • Oversight of the Agency clinical program, lead and manage the day-to-day operations of the department.
    • Oversee and manage clinician staff and monitor the clinical performance of the department.
    • Work with HR in hiring qualified staff, lead orientation and competency assessment of clinical staff.
    • Assist the Administrator in planning the overall development and administration of the Agency as set forth in the policies and procedures relating to the agency’s programs.
    • Reviews and evaluates existing policies and practices to determine if current methods provide the means for the staff to carry out their responsibilities and achieve projected goals.
    • Utilizes the data reports to quantify agency’s performance against the national norms. Recommends to the Administrator methodology to change outcomes and improve scores.
    • Oversee the clinical record review summaries for the provider to present at the QAPI committee.
    • Assures that established PI committees maintain the goal of meetings per calendar quarter.
    • Assists the Administrator in planning and conducting agency Committee meetings.
    • Receives and acts to resolve complaints subject to the agency’s grievance procedure.
    • Treat in the field if needed.
    • Maintaining compliance with Accreditation status, Home Health Care Federal and State regulations.
