CLINICAL OUTCOMES MANAGER manages the collection and analysis of clinical outcomes data to develop clinical process improvement initiatives. Oversees staff who gather data on performance metrics and facilitates the review of clinical program effectiveness. Being a CLINICAL OUTCOMES MANAGER provides advanced analyses and reports on clinical outcomes data. Evaluates and summarizes patient data to ensure that care is provided in accordance with clinical guidelines and organizational standards. Additionally, CLINICAL OUTCOMES MANAGER identifies areas for improvement and assists leaders/physicians in developing strategies to improve performance. Requires a bachelor's degree in nursing or healthcare related area. May require a Registered Nurse (RN) license. Typically reports to a director. The CLINICAL OUTCOMES MANAGER typically manages through subordinate managers and professionals in larger groups of moderate complexity. Provides input to strategic decisions that affect the functional area of responsibility. May give input into developing the budget. To be a CLINICAL OUTCOMES MANAGER typically requires 3+ years of managerial experience.     POSITION SUMMARY:

    A Registered Nurse that assists the Administrator with the supervision of hospice services to patients in their place of residence in accordance with physician orders and strives to provide the highest quality of care. The Clinical Manager is available at all times during operating hours (or appoints a similarly qualified alternate) and participates in all activities relevant to professional services furnished, including the development of qualifications and assignment of personnel.

    QUALIFICATIONS:
    • Registered Nurse with current licensure to practice in the state where hospice is located

    RESPONSIBILITIES:
    • Supervise, orient and provide ongoing training for clinical field teams.

    o Organize and lead nursing meetings and in-service programs for field clinicians.

    o Identify needs and provide additional training to clinical team based on clinical audits, outcome scores, reports and tandem visits. Reports received from software

    o Training activities and responsibilities include:

    o Provide initial Clinical Orientation for new field nursing staff.

    o Coordinate and deliver training and testing

    o Train Supervisory visit nurses on proper assessment of the unskilled patient and appropriate referral to other services.

    o Train nurses in the field on admissions, skilled visits, and skilled procedures.

    o Perform tandem visits with nurses to provide support and training and to assess competency– Oasis, wound care, trach care, IV’s, etc. Perform yearly and as needed tandem competency visits and on site supervisory visits to supervise field staff.

    o Complete annual reviews and counseling of nursing staff.

    • Provide clinical support and direction to office and field teams.

    o Provide support to clinicians, office and field teams on clinical issues.

    o Cover any cases that cannot be filled; back up clinical coverage.

    o Ensure ongoing communication with all members of the clinical team.

    • Provide clinical support and direction to management and corporate teams and assure that overall daily clinical care is safe, appropriate and compliant with state, federal and nursing regulations and standards.

    o Coordinate with Coding and Clinical Review Nurse to assess cases identified as having potential inappropriate episode utilization or premature/inappropriate re-cert or discharge.

    o Coordinate and facilitate on site state and program specific compliance surveys’ and develop plan of corrections as needed. Assure that plan of correction is completed.

    o Assist Administrators with coordination and implementation of QAPI.

    o Collaborate with entire Corporate Clinical Team and assist with onsite clinical follow thru.

    o Assist with ADR’s that require clinical intervention and documentation.

    o Monitor reports in EHR software and review to assure proper utilization of skilled services while still ensuring the patients receive proper and appropriate care.

    o Monitor reports using EHR software and QAPI to improve data collection resulting in more accurate reflection of client improvement, and to improve outcomes reported to the State by Oasis data collection.

    • Assure compliance with State, Federal and local laws and Board of Nursing Regulations. Monitor and implement plans to achieve acceptable clinical outcomes. Assure compliance with Clinical Policy and Procedure.
    • Perform audit/investigation of Adverse Events and report findings to Administrator.
    • Performs data collection and assists with the Agency’s ongoing Quality Assessment and Performance Improvement (QAPI) program.
    • Oversee and maintain clinical supply utilization and approve orders outside of threshold/protocol.
    • Assists in the coordination of interdisciplinary team services, patient care, and referrals.
    • Assures the development, implementation and updates of the individualized patient plans of care.
    ADDITIONAL RESPONSIBILITIES:
    • The Clinical Manager is expected to keep their CPR and RN licensure current.
    • The Position is also expected to attend mandatory in-services and meetings, which may include travel.
    • Supportive of individual and company performance improvement initiatives.
    • Maintain confidentially with employee and company sensitive information.
    • Comply with accepted professional standards and practices.
    • Complete projects and assignments as assigned by Hospice Administrator.
    • The Clinical Manager is expected to participate in appropriate continuing Education as required by the Agency.
