CLINICAL OUTCOMES MANAGER manages the collection and analysis of clinical outcomes data to develop clinical process improvement initiatives. Oversees staff who gather data on performance metrics and facilitates the review of clinical program effectiveness. Being a CLINICAL OUTCOMES MANAGER provides advanced analyses and reports on clinical outcomes data. Evaluates and summarizes patient data to ensure that care is provided in accordance with clinical guidelines and organizational standards. Additionally, CLINICAL OUTCOMES MANAGER identifies areas for improvement and assists leaders/physicians in developing strategies to improve performance. Requires a bachelor's degree in nursing or healthcare related area. May require a Registered Nurse (RN) license. Typically reports to a director. The CLINICAL OUTCOMES MANAGER typically manages through subordinate managers and professionals in larger groups of moderate complexity. Provides input to strategic decisions that affect the functional area of responsibility. May give input into developing the budget. To be a CLINICAL OUTCOMES MANAGER typically requires 3+ years of managerial experience.     Position Summary: The Absolute Home Health Clinical Manager is responsible for the planning, coordinating, and delivery of quality skilled services. This position will ensure efficient and effective utilization management by client needs, physician orders, and payer requirements. The position reports to the Agency Administrator/Administrator/Director of Operations who will also prepare and present performance evaluations annually or as indicated.
    Primary Duties and Responsibilities:
    • This registered nurse is an employee of the agency who supervises and directs the skilled nursing and other therapeutic services of the agency
    • Direct responsibility for delivery of quality care that meets client-specific needs and complies with program/payer rules and physician orders.
    • Responsible for efficient and effective utilization management by client needs, physician orders, and payer requirements.
    • Facilitates communication and coordination of care between clients, families, caregivers, physicians, and other service providers as necessary. Including participating in on-call rotation.
    • Assures efficient coordination of caregivers through case conferences and scheduling processes.
    • Assures current and accurate clinical records in compliance with documentation requirements and program-specific rules for coverage and qualifying criteria. Assures clinical records are maintained in their original state and not improperly altered or fabricated. Plans and assures adherence to budgetary guidelines, and exercises appropriate expense control management.
    • Ensures care delivery meets the needs of the patients and follows professional practice standards
    • Implements appropriate hiring, orientation, assignment, and development of staff is available at all times during operation hours, readily available on the premise or via telecommunication
    • Participates in all activities relevant to services delivery and professional services furnished
    • Observes, supervises, counsels, and disciplines staff effectively, including conducting annual performance reviews of employees with input from the Administrator/Director of Operations or the Governing Body
    • Participates in Administrative on-call
    • Monitors compliance with applicable Federal, State, and local laws, professional standards and principles, established policies and procedures, and the plan of care
    • All the duties assigned within the scope of the licensure, by the Administrator/Director of Operations or the Governing Body
    Education:
    • Graduate of an approved school of professional nursing
    • Licensed in the State of Ohio, in good standing to practice as a registered nurse
    • Basic knowledge of Microsoft Office and Microsoft Excel
    Training and Experience:
    • Two years of home care experience, with at least one year of management experience
    • Management experience including but not limited to, supervision, disciplinary actions, and staff development
    • Knowledge of Medicare/Medicaid Home Care Regulations
