CLINICAL OUTCOMES MANAGER manages the collection and analysis of clinical outcomes data to develop clinical process improvement initiatives. Oversees staff who gather data on performance metrics and facilitates the review of clinical program effectiveness. Being a CLINICAL OUTCOMES MANAGER provides advanced analyses and reports on clinical outcomes data. Evaluates and summarizes patient data to ensure that care is provided in accordance with clinical guidelines and organizational standards. Additionally, CLINICAL OUTCOMES MANAGER identifies areas for improvement and assists leaders/physicians in developing strategies to improve performance. Requires a bachelor's degree in nursing or healthcare related area. May require a Registered Nurse (RN) license. Typically reports to a director. The CLINICAL OUTCOMES MANAGER typically manages through subordinate managers and professionals in larger groups of moderate complexity. Brief Summary:
Under the supervision and guidance of the Chronic Disease Management Provider, the Chronic Disease Management Outcomes Coordinator is responsible for the collaborative process of assessment, planning, implementation, and evaluation of care coordination for patients. The coordinator advocates for options and services to meet and individual’s and family’s comprehensive health needs through communication and available resources to promote quality, cost effective outcomes. The coordinator demonstrates knowledge and skills and understands the unique aspects of the patients served. In addition, perform nursing care and case management for patients with the collaboration of Samuel Simmonds Memorial Hospital (SSMH) Chronic Disease Management providers in the clinic, procedural and inpatient setting including but not limited to acute care and swing bed admissions.
