Clinical Outcomes Director directs in-depth analysis of clinical outcomes data to develop clinical process improvement initiatives. Oversees and leads staff who gather data on performance metrics and facilitates the review of clinical program effectiveness. Being a Clinical Outcomes Director creates, defines, and implements performance metrics to review program effectiveness. Evaluates patient data to ensure that care is provided in accordance with clinical guidelines and organizational standards. Additionally, Clinical Outcomes Director identifies opportunities and provides recommendations to improve program delivery, patient outcomes, and operational efficiency. Requires a bachelor's degree in nursing or healthcare related area. May require a Registered Nurse (RN) license. Typically reports to senior management. The Clinical Outcomes Director manages a departmental sub-function within a broader departmental function. Creates functional strategies and specific objectives for the sub-function and develops budgets/policies/procedures to support the functional infrastructure. To be a Clinical Outcomes Director typically requires 5+ years of managerial experience. Deep knowledge of the managed sub-function and solid knowledge of the overall departmental function.     A DAY IN THE LIFE OF OUR HOSPICE EDUCATION AND OUTCOMES COODINATOR (RN)

    As our Hospice Education and Clinical Outcomes Coordinator, you arrive each day ready to take our hospice services above and beyond. The primary function is to assume responsibility for administrative tasks and leadership functions for assigned office(s) in coordination with Administrator. You manage our office and clinical operations in order to meet the needs of our patients and help them live each day to the fullest. Ensuring our patient's needs are met, you coordinate the daily workload and 24-hour services. Working with the HR department, you assist with recruiting, hiring, and training new employees. You act as a resource for our field staff, investigate and resolve all reported incidents, conduct performance evaluations, and develop performance improvement plans as well as educate employees on infection control and how to recognize and report abuse. Other duties include monitoring the KPI, the QAPI program, and compliance. You will also be responsible for reviewing HIPAA complaints and work with CM and Administrator to help resolve the complaints.

    QUALIFICATIONS

    • Oklahoma RN license
    • 4 years of nursing experience
    • 2 year of hospice leadership roles.
    • Ability to obtain CPR certification within 60 days of hire.
    • Knowledge about the Medicare guidelines governing Hospice agencies.
    • Valid Oklahoma driver's license and vehicle insurance
    • Basic computer skills
