Clinical Outcomes Analyst collects clinical outcomes data and performs analysis in support of clinical process improvement initiatives. Gathers data on performance metrics to facilitate the review of program effectiveness. Being a Clinical Outcomes Analyst evaluates and summarizes patient data to ensure that care is provided in accordance with clinical guidelines and organizational standards. Identifies areas for improvement and communicates findings to leadership. Additionally, Clinical Outcomes Analyst requires a bachelor's degree in nursing or healthcare related area. May require a Registered Nurse (RN) license. Typically reports to a manager. The Clinical Outcomes Analyst work is closely managed. Works on projects/matters of limited complexity in a support role. Brief Summary:
Under the supervision and guidance of the Chronic Disease Management Provider, the Chronic Disease Management Outcomes Coordinator is responsible for the collaborative process of assessment, planning, implementation, and evaluation of care coordination for patients. The coordinator advocates for options and services to meet and individual’s and family’s comprehensive health needs through communication and available resources to promote quality, cost effective outcomes. The coordinator demonstrates knowledge and skills and understands the unique aspects of the patients served. In addition, perform nursing care and case management for patients with the collaboration of Samuel Simmonds Memorial Hospital (SSMH) Chronic Disease Management providers in the clinic, procedural and inpatient setting including but not limited to acute care and swing bed admissions.
