WGH is seeking a full-time Clinical Wellness Nurse Coordinator to join our growing organization. The Clinical Wellness Nurse Coordinator provides professional nursing care and collaborates with the provider care team to facilitated, provide, and promote successful preventive care visits and screenings to reduce the incidence and severity of chronic diseases and acute illnesses. This team member will promote effective partnerships between patients, families, nurses, physicians, chronic care nurse managers and other clinical disciplines to coordinate care for patients and provide a strategic approach to identify and mitigate preventable and progressive disease processes.
The Clinical Wellness Nurse Coordinator will collaborate with team leaders and directors to implement effective internal tracking processes to capture results, ensure accurate and timely medication acquisition, and influence patients towards follow-through of any preventive screening tests, such as colonoscopies, mammographies, low-dose CT scans, immunizations, and other screenings, as well as any follow-up appointments. This team member will also develop a process to track annual wellness visits (AWV) scheduling and ensure that patient records are reviewed appropriately to identify care gaps prior to visits with their provider.
Additionally, the Clinical Wellness Nurse Coordinator will provide clinical health coaching interventions to motivate patients and families toward successful self-management of preventable illness, and in collaboration with the provider, develop a care plan based on mutual goals. This team member will facilitate access to appropriate medical and specialty providers and participate and monitor QI metrics or initiatives as they relate to preventive medicine.
