Clinical Nurse Specialist - Home Care jobs in Minnesota

```Duties:```
- Provide skilled nursing care to patients in their homes
- Perform assessments and develop individualized care plans
- Administer medications and treatments as prescribed by healthcare providers
- Monitor and evaluate patients' conditions and report any changes to the healthcare team
- Assist with activities of daily living, such as bathing, dressing, and meal preparation
- Coordinate with other healthcare professionals to ensure continuity of care

- Valid nursing license (RN or LPN)
- Experience in home care or a related field preferred

-Private Duty RN-LPN Stillwater-10,000 retention bonus per year- $40.00 to $45.00 per hour for RN and $35.00 per hour for LPN.

-Work with a 15 year old boy who loves sports and the outdoors.

-Come join our care team for our teenage son who is in need of support by RN or LPN. 8am to 4pm shifts 4 - 5 days per week. No weekends, nights or holidays.
