    Primary Purpose Provides specialized integrated clinical pharmacy services for an assigned area. Provides leadership in education and practice of evidence based medication use and optimal patient outcomes.

    Minimum Specifications

    Education

    Must have a Bachelor's Degree in Pharmacy or Pharm. D. degree from an accredited school of pharmacy

    Experience

    Successful completion of an ASHP-accredited post-graduate year one (PGY1) residency training program and post-graduate year two (PGY2) residency training program or fellowship

    Meets Parkland standards of an ASHP-Qualified Residency Preceptor within one year of employment.

    Prior experience working in assigned practice area preferred.

    Equivalent Education and/or Experience

    - additional certification(s) or years of relevant clinical pharmacist experience would be evaluated for equivalent if no prior residency training.

    Certification/Registration/Licensure

    - Texas Pharmacist License or eligible for Texas Pharmacist License; must obtain licensure within 90 days of employment.

    - Prefer national certification in any Pharmacy specialty such as Board of Pharmacy Specialties (BPS) certifications in ambulatory care (BCACP), cardiology (BCCP), critical care (BCCCP), geriatric (BCGP), infectious diseases (BCIDP), nutritional support (BCNSP), oncology (BCOP), pediatric (BCPPS), pharmacotherapy (BCPS), or psychiatric (BCPP) pharmacy.

    Must possess knowledge of drugs, procedure, equipment, common disease states and patient care monitoring within the assigned care areas.

    Must possess good oral and written communication skills.

    Must possess good organizational skills.

    Must have proficient computer skills.

    Must possess extensive mathematical skills.

    Responsibilities

    1. Evaluates, monitors, &/or modifies patient pharmacotherapy to ensure effective and safe patient care. Designs, implements, and manages patient drug regimens based on evidence based medicine, formulary compliance, and individual patient needs in collaboration with medical teams or practicing under medication therapy management protocols with physicians.

    2. Demonstrates competency/knowledge in computer entry/verification of medication orders/order processing/order preparation including specialty orders. Submits/reviews standardized medication order/sets according to clinical guidelines to improve ordering safety, efficiency, and patient/performance outcomes.

    3. Participates in the development, management, education, & implementation of cost effective, evidence based therapies or protocols, clinical practice guidelines. Monitors drug expense in practice area. Reviews Non-formulary use. Maintains provider relationships and gathers feedback regarding historical and future drug pipeline expense trends. Facilitates/participates in P&T subcommittee activities related to practice areas.

    4. Actively participates in departmental &/or hospital committees/meetings as medication management authority in specialized practice area. Performs inspections, research, audits, MUE, &/or performance improvement activities in support of clinical practice and measurement of best practices/regulatory standards. Works with medical & nursing staff to enhance patient outcomes, safety, and regulatory standards.

    5. Educates and communicates effectively with patients on medication therapies to ensure that patients are informed of proper drug usage and knowledge of potential side effects. Leverages experience with various patient populations, literacy levels, and all available patient information both documented & through patient interview to individualize medication regimens and adherence tools communicated to the patient/caregiver.

    6. Provides a lead role in answering complex drug information requests and applies outcomes to general and specific patient situations. Consults scientific publications for practical clinical applications of the data to practice area. Objectively evaluates medical and advertising claims for medication products. May be required to staff or be on-call to respond to patient care medication related questions as the resident expert when needed.

    7. Provides mentoring/training/education to pharmacy students, residents, pharmacists, technicians through individual patient cases with more formal education regarding disease or drug therapy, or formal education programs such as CE or staff development programs. In addition, educates providers, nurses, residents, students, patients, and the public regarding drug therapy.

    8. Attends, participates, & provides feedback in journal club, in-services, resident topic discussions, and resident/student case presentations

    9. Documents interventions for resolved prescribing, formulary & therapeutic issues. Identifies & discusses adverse drug events with the care team & documents safety opportunities. Suggests & implements with pharmacy leadership new processes, systems or procedures to enhance patient safety & the prevention of adverse events or errors in any step of the medication management process.

    10. Maintains a positive working relationship with job related contact such as management, healthcare professionals, staff, and other employees in order to improve overall pharmacy services and to ensure that all job contacts are pleasant and productive. Effective in communication of care issues and professional interactions with peers & associates.

    11. Maintains knowledge of applicable rules, regulations, policies, laws and other guidelines that impact the Pharmacy profession. Coordinates with other departments, physicians, and/or nurses to develop effective internal controls that promote adherence to applicable state/federal laws, program requirements of accreditation agencies, and federal, state, and private health plans. Seeks advice and guidance as necessary to ensure proper interpretations.

    12. Stays abreast of the latest developments, advancements, and trends in the field of pharmacy & medicine by attending seminars/CE, reading journals, active participation in professional organizations &/or maintaining licensure & additional certifications. Integrates knowledge gained into current work practices.

    13. Conducts/participates in research which impacts delivery of quality care. This can be IRB approved or internal PI projects. Promotes Parkland Health on a local and/or national level including publications, presentations, research collaborations, projects, inservices, volunteer efforts, recruiting, disaster response, etc.
